The spine is also one of the human regions in which a surgery is most frequently performed though. Spinal fusion is one of the most common surgical techniques for treating spinal disease. However, complications of spinal fusion due to its excessive stiffness have been reported. Although new spinal implants such as flexible rods and artificial discs were developed and have been recently used to avoid the complications of spinal fusion, there are only few studies on analyzing biomechanical characteristics of these implants. In this study, biomechanical performance of new spinal implants such as flexible rods and artificial discs were evaluated using virtual test technology based on finite element analysis.
Three dimensional finite element models of the lumbar and the cervical spines were developed using computed tomography images of a male subject, anatomical information such as attachment points of ligaments, and material properties of bones and implants from previous studies. The models were validated by comparing their stiffness to the stiffness reported in previous studies. Spinal fusion using dynamic stabilization and total disc replacement using artificial disc were virtually performed in the validated models according to preoperative surgical plans made by orthopedic and neurosurgical surgeons. The virtual tests were performed to evaluate biomechanical performance of spinal implants. For the tests, bottom of each implanted spine model was fixed, and moments in various directions were applied on the superior plate of each model with follower load.
The maximum von-Mises stress concentrated on the neck region in the pedicle screw of dynamic stabilization system decreased as flexibility of rod increased. Accordingly, the fracture risk of the system also decreased. However the maximum stress on the flexible rod and its fracture risk increased. When every artificial disc was inserted in the lumbar spine, range of the flexion motion of implanted level was similar with the same level of the model of an intact lumbar spine. However, excessive extension motion generated in every artificial inserted level leads to increase of contact force on the facet joint. On the aspect of fracture risk of an artificial disc, an artificial disc of un-constraint concept had an advantage comparing with one of semi-constraint concept. When an artificial disc was inserted in the cervical spine, motions of the implanted level in all direction were higher than motions of the same level of intact cervical spine. In addition, high range of motion leads to increase of contact forces on the facet joints and stresses of major ligaments. The movement of an artificial disc to posterior direction made flexion motion increased.
From the results of this study, various complications which might occur in spinal surgeries such as spinal fusion and total disc replacement could be comprehended. These results could be helpful to select a spinal surgical method and a spinal implant, and to make a preoperative plan for a spinal surgery. Furthermore, the developed finite element models of the lumbar and the cervical spines, and virtual test technology using the developed models could be very useful to prove the effect of a surgical technique, to analyze and improve a present spinal implant, and to develop new surgical methods and spinal implants.
