Purpose: The purposes of this study were to investigate Hill-Sachs lesion on MR arthrography in patients who present with Bankart lesion at arthroscopy following anterior shoulder dislocation, and correlate the extent of Hill-Sachs lesion in regard to the extent of Bankart lesion proliferation at the time of identification. In addition, the relationships that extent of both lesions and the frequency of shoulder dislocations were also investigated.
Materials and Methods: Eighty six MR arthrograms of patients who received arthroscopic Bankart repair were retrospectively analyzed. All patients had a history of anterior shoulder dislocation. On MR arthrography, a notched defect or flattening on the posterorlateral portion of the humeral head was regarded as Hill-Sachs lesion, and the presence or absence of Hill-Sachs lesion was evaluated. If it was present, the longest surface length of Hill-Sachs lesion was measured on the contiguous axial MR images. The length of Bankart lesion was recorded on the basis of conventional clock-face description from arthroscopic findings. Medical records were reviewed for the frequency of shoulder dislocations. The measurements of the longest surface length of Hill-Sachs lesion were correlated with the length of Bankart lesion. And the relationship between the longest surface length of Hill-Sachs lesion and the frequency of shoulder dislocations, the relationship between the length of Bankart lesion and the frequency were also assessed, using Spearman?s rank test, respectively.
Results: Hill-Sachs lesions were identified in 90.7% (78/86) on MR arthrography in patients with Bankart lesion at arthroscopy, and the mean of its longest surface length was 18.8 mm (range, 9.3 ? 29.6 mm). The mean extent of Bankart lesions was 4.25 o?clock extent (range, 1-6). The mean frequency of shoulder dislocations was 24.5 times (range, 1-100). The longest surface length of Hill-Sachs lesion was significantly correlated with the length of Bankart lesion at arthroscopy (? = 0.37, p = 0.001); the extent of both lesions was not correlated with the frequency of dislocations.
Conclusion: Hill-Sachs lesion was very common in patients with Bankart lesion following anterior shoulder dislocation. The extent of Hill-Sachs lesion was positively correlated with the extent of Bankart lesion. This result suggests that the larger Hill-Sachs lesions, the severer Bankart lesions that were present. Therefore, the coexisting Hill-Sachs lesion should be evaluated in patients with Bankart lesion. Accurate detection and evaluation of Hill-Sachs lesion may be helpful in planning proper treatment. However, the extent of both lesions was not significantly correlated with the frequency of shoulder dislocations.
