Background : Chronic obstructive pulmonary disease(COPD) is common disorder in clinic and reported as 4th cause of death in the world. Recently COPD has been increased in Korea and the costs of COPD also increase. The purpose of this study was to describe the medical costs incurred by patient according to the severity of COPD.
Methods : A total of 440 outpatients and 141 inpatients with COPD(chronic bronchitis, emphysema, other COPD) were enrolled retrospectively from Kyung Hee University Medical Center for 1 year. Direct costs were calculated for patients with confirmed COPD according to the degree of severity of airflow obstruction.
Results : In outpatients, mean per-person number of our hospital respiratory clinic visit for 1 year has no relationship between the groups of severity of COPD. Mean per-person direct medical expenditures for respiratory clinic of our hospital increased significantly as the severity of airflow obstruction was raised(StageⅠ: 174,212 won, StageⅡ: 172,996 won, StageⅢ: 260,297 won, StageⅣ: 309,592 won). Mean per person hospitalization days which was related to COPD tended to increase by the severity of airflow obstruction but showed the fewest days in StageⅣ(StageⅠ: 9.91 day, StageⅡ: 17.09 days, StageⅢ: 25.62 days, StageⅣ: 11.67 days). Similarly, mean per person hospitalization costs which was related to COPD tended to increase by the severity of airflow obstruction but showed the lowest costs in Stage Ⅳ(StageⅠ: 5,114,112 won, StageⅡ: 7,862,746 won, StageⅢ: 9,591,564 won, StageⅣ: 3,932,279 won).
Conclusion : Mean per-person direct medical costs of outpatients with COPD were associated with the severity of airflow obstruction. Mean per-person hospitalization days and costs also tended to increase as the severity of airflow obstruction was raised especially in stageⅢ. Therefor, it is important that we prevent the progression of COPD, especially stageⅢ in order to reduce total costs of COPD in Korea.
