Beck Depression Inventory (BDI)와 State- Trait Anxiety Inventory (STAI), Korean asthma related quality of life (KAQLQ)의 설문 조사를 이용하여 불안, 우울, 삶의 질을 분석하였다. 결과 : 대상 환자는 전체 120 명으로 평균 연령은 55 ± 1 세였으며, 여자가 65 %를 차지하였다. 천식 중증도 분류상 경증 천식군이14 명 (12%), 중등증 천식군이 88 명 (73%), 중증 천식군이18 명 (15%)이었으며, 120 명 중 81 명(68%)이 잘 조절되는 군이었다. BDI 21점을 기준으로 우울을 진단하였을 때 우울은 천식의 중증도에 따라 경증 천식군3 명(21%), 중등증 천식군9 명(10%), 중증 천식군6 명(33%)으로 통계학적으로 유의한 차이를 보였으며(p=0.004), 삶의 질 평가에서도 우울이 동반된 천식 환자에서 유의하게 낮았다(p=0.034). 조절군에서 우울 환자가 8 명(10%)으로 비조절군 10 명(26%)보다 유의하게 낮았으며(p=0.024), 삶의 질 역시 비조절군의 평균값인 35.103 점(18-72)에 비해 조절군에서 59.951 점(29-75)으로 높은 평가 점수를 보였다(p=<.0001). 우울을 동반한 천식 환자군에서 중증 환자가 많았고, 불안을 동반한 경우가 많았다. 결론 : 우울은 천식의 중증도와 조절에 밀접한 관계가 있음을 알 수 있으며, 천식환자에서 조기에 우울증 및 불안 장애, 삶의 질을 평가하고 위험 요인을 분석하는 것이 치료 효과를 증진시키는데 기여할 것으로 생각된다.

This study assessed the presence of depression, anxiety and asthma related quality of life and evaluated its impact on severity and control of asthma. Methods : We assessed the severity of asthma based on current medication by GINA guideline 2004. The patients classified into the controlled group and the uncontrolled group which include partly and uncontrolled asthma patients by GINA guideline. Patient-reported depressive symptoms, anxiety and asthma related quality of life were evaluated using Beck Depression Inventory (BDI), the State-Trait Anxiety Inventory (STAI) and Korean asthma quality of life (KAQLQ). Results : One hundred and twenty patients were included in this study (mean age, 55±1 years; 65% women). Among 120 patients, 14 (12%) of patients were classified as mild asthma, 88 (73%) as moderate asthma and 18 (15%) as severe asthma. Thirty nine (33%) of the 120 patients were uncontrolled. The depression and asthma related quality of life showed the difference according to severity of asthma (P=0.004, P=0.034, respectively). The prevalence of depression were lower (10% vs. 26%, P=0.024) and the asthma related quality of life were higher (59.951(29-75) vs. 35.103(18-72), P≤0.001) in the controlled group than in the uncontrolled group. Severe asthma, higher STAI-X-2 score and lower asthma related quality of life were associated with depression in asthma (P=0.004, P<0.001, P=0.002, respectively). Conclusions : Psychological factors such as anxiety and depression are strongly associated with severity and control of asthma. Therefore, screening and management of depression is needed in patients with asthma.
