방법: 본 연구는 마산삼성병원 경남권역응급의료센터에서 이루어졌으며, 2007.08.01~2009.12.31까지 총 1,500명의 외상환자 대상으로 NEDIS 및 진료기록 등으로부터 후향적으로 자료가 수집되었다. 1,500명의 외상환자 자료 중에서 앞서 내원한 1,000명의 환자군은 모형개발을 위한 자료군에 할당되었고 그 이후에 내원한 500명의 환자군은 모형검증을 위한 자료군에 배정되어 분석이 이루어졌다. 모형개발단계에서 여러 개의 모형을 개발한 후 이들 모형들에 대한 적합성 검정을 모형검증을 위한 환자군을 대상으로 분할표, Hosmer- Lemeshow 검정 및 Receiver Operating Characteristic (ROC) 곡선 분석 등의 방법을 이용하여 최적의 모형을 선정한 후 이 모형들과 Trauma and Injury Severity Score (TRISS) 모형을 비교하였다.
결과: 개발단계에서 6개의 사망예측모형이 도출되었다. 이 중에서 3개만이 모형검증을 위한 자료에서 적합성 검정 결과에 의거하여 최종모형으로 결정되었는데, 이들 모형들은 나이, AVPU scale, 수축기 혈압, 맥박수, 호흡수. 동맥혈 pH, 혈청단백질 수치 등의 변수들을 포함하였다. 첫 번째 모형(NEDIS모형)은 NEDIS에서 수집이 가능한 나이, AVPU scale, 수축기 혈압, 맥박수 및 호흡수 만으로 구성되었고, 두 번째 모형(ABGA+모형)은 NEDIS모형에서 맥박수가 빠지고 동맥혈 pH가 포함되었으며, 마지막 모형(LAB+)은 ABGA+모형에 혈청단백질 수치가 포함되었다.
TRISS 모형과 세 모형의 정확도는 각각 91.8%, 90.8%, 91.8% 및 90.8%로 서로 차이가 없었고, ROC 곡선 밑면적은 각각 0.954, 0.895, 0.917 및 0.922로 NEDIS 모형의 변별력이 낮았으며, Hosmer-Lemeshow의 검정에서 TRISS를 제외하고 세 모형은 소집단별 사망예측이 정확하였다. 그러나 revised trauma score < 7.0 이거나 injury severity svore > 15인 중증외상 환자만을 대상으로 한 결과는 TRISS와 세 모형의 ROC 곡선 밑면적이 0.866~0.888로 서로 차이가 없었으며 정확도 또한 77%~80%로 차이가 없었다. 그러나 Hosmer- Lemeshow의 검정에서 네 모형 모두 소집단 사망 예측이 부정확하였고 그 정도는 TRISS가 가장 심하였다.
결론: 본 연구에서 도출된 외상환자 생존 예측모형모형들은 현재 가장 많이 쓰이는 TRISS에 비해 변별력과 분류 정확도에서 차이를 보이지 않고 오히려 소집단별 예측 일치도는 더 우수하였다.

Objective: This study aims to develop new logistic regression model to predict mortality in trauma patients using National Emergency Department Information System (NEDIS) and other physiologic data to be obtained easily in Emergency Department.
Method: This study was conducted in Kyungnam Wide Regional Emergency Center (WREC) affiliated with Masan Samsung Hospital. Data were obtained via retrospective record review of 1,500 consecutive trauma patients visiting Kyungnam WREC from Aug 2007 to Dec 2009. Data from 1,000 patients were assigned to development dataset and others were done to validation dataset. After several new logistic regression models were developed from development dataset, goodness-of-fits of these models were evaluated from validation dataset using classification tables, Hosmer-Lemeshow X2 test and Receiver Operating Characteristic (ROC) curve analysis.
Result: Six logistic regression models were derived from development dataset. Of which, three models were decided as final models based on goodness-of-fit test, and included age, AVPU scale, systolic blood pressure, pulse rate, respiratory rate, arterial pH and serum protein level. One model was named as NEDIS model, because it included NEDIS data only. The other models were named as ABGA+ and LAB+ model because they included arterial pH or/and serum protein level in addition to NEDIS data. The accuracies of the TRISS and those models were 91.8%, 90.8%, 91.8%, and 90.8%, respectively, and also the areas under ROC curve (AUC) of those models were 0.954, 0.895, 0.917, and 0.922, respectively. Those models except the TRISS had good subgroup mortality prediction in Hosmer-Lemeshow X2 test. In major trauma patients which had revised trauma score less than 7 or injury severity score more than 15, The TRISS and those models had the AUCs of 0.866~0.888, the accuracies of 77.9%~80.1%, and poor subgroup mortality prediction.
Conclusion: Compared with the TRISS, new models to predict mortality in trauma patients had same accuracies and discriminative powers, and good subgroup mortality prediction. Therefore the authors recommend these models as tools for evaluating trauma outcome.
