본 연구는 2009년 4월 폐의약품 회수처리 시범사업 실시 후 대구시민의 가정에서 의약품 폐기 실태 파악, 회수처리 시스템에 대한 인식도, 인식 후 행태 및 반납경험과의 관련요인을 조사하고 현 시스템의 정착상태와 개선사항에 대해 일반인과 약사 두 집단의 인식을 조사하고자 1개 상급종합병원의 내과 또는 소아과를 방문한 환자나 보호자 중 기혼여성 232명과 개국․관리약사 100명을 대상으로 면접설문조사를 하였다. 전체 232명 중 가정 내 복용하지 않은지 3개월이 경과된 미사용약의 보유는 전문의약품 62.9%, 일반의약품 59.9%로 증상완화에 따라 임의로 복용중단을 하는 감기약, 소염진통제, 소화제 등이었고, 수제의 경우 약 40%가 싱크대 혹은 화장실 변기로 폐기, 그 외 제형은 약 70%가 종량제 쓰레기봉투에 폐기하는 것으로 조사되었다. 회수처리 시스템에 대한 인지도는 들어본 적이 있다 43.6%, 들어 본 적이 없다 56.4%이나 인지한 101명 중 42.5%는 종전과 같이 폐기행태를 유지함을 알 수 있었고, 전체 232명 중 10.8%만이 약국으로 미사용약을 반납한 경험을 갖고 있었다. 회수처리 시범사업에 대한 정착상태는 일반인 77.6% 약사 34.0%는 아니다, 일반인 17.2% 약사 52.0%는 그저 그렇다고 응답하여 두 집단 간에 유의한 차이(p<0.001)를 나타내었다. 그리고 약국의 수거함 비치현황은 91.0%이나 일반인이 수거함을 본 적이 있다고 응답한 경우는 19.4%로 낮았고, 인지경로의 71.3%가 방송매체 등에 의한 홍보효과로 조사된 바와 같이 수거함의 디자인과 크기, 잠금장치 등에 대한 보완과 약국 안에서도 일정한 장소를 지정함으로서 간과되지 않도록 해야 할 것이다. 활성화 방안의 하나로 수거함의 추가 비치장소에 대해 일반인과 약사 두 집단 간에 유의한 차이(P<0.001)를 나타내었으나 폐기의 효율성과 약품의 안전성을 고려 시 병원 내 약국을 검토 할 필요가 있으며 약국에서 제도상 개선되어할 가장 중요한 사항은 수거함의 규칙적, 정기적 회수가 필요함을 알 수 있었다. 폐의약품 회수처리 시스템의 한 주체가 되는 약사들의 적극적 개입으로 미사용량의 발생을 줄이고 회수처리에 대한 정확한 정보제공과 PharmEcovigilance의 역할을 수행할 수 있도록 약사들의 체계적, 전문적 교육이 필요할 것이며, 본 연구에서 반납경험에 유의한 변수는 회수처리 시스템에 대한 인식도와 환경오염에 대한 인식도(p<0.01)이었다. 그러므로 지속적, 장기적인 홍보 활동으로 인지도를 높이고, 환경 상 노출로 수중 생태계 교란 뿐 아니라 하수처리 과정에서 완전히 제거되지 않은 의약품은 결국 식수원에 도달할 수 있다는 ‘만성적 영향’을 홍보, 교육하여 인지 후 행태변화를 유도하고 지속함으로 폐의약품 회수처리사업이 잘 정착되도록 해야 할 것이다.

There has been an enhanced international awareness that improper disposal of unwanted and unused medications may have a detrimental effect on the environment. To prevent hazards and environmental pollution, the Waste Medication Collection and Disposal System was set up in April 2009, Korea. This survey was performed to examine disposal practices and to evaluate recognition by consumers and pharmacists in Daegu 10 months after the implantation of the system. The survey was conducted via a questionnaire with 232 married women and 100 pharmacists. About 60% of the respondents indicated that there were leftover, unused or unwanted prescription and nonprescription medications present in their homes. Depending on the formulation type, 70.4% of the respondents disposed of their solid-type medications in the garbage and about 40% of the respondents disposed of their liquid-type medications by flushing them down the toilet or sink. A total of 43.6% were aware that the correct process of disposal was to return unused medications to the pharmacy, but only 10.8% had ever returned unused medication. This experience rate differ significantly depending on perceived environmental awareness and knowledge of collection system. 93.0% of the pharmacists were aware of the system, 91.0% of them had a collection box installed. However, only 19.0% of them experienced returning of unused medications more than once a week and 46.0% of them provided information from time to time. Also, more pharmacists recognized the serious risks medications pose to the environment than general citizens. In conclusion, this study suggests that the public should be educated regarding proper disposal practices and environmental awareness via mass media, repeatedly and for a long period of time so that the message can reach everyone. Secondly, collection boxes should be designed to be eye-catching and unique so that the public can recognize them easily, and relate them to their purpose. Finally, pharmacists should be actively involved in reducing the waste of medications.
