The purpose of this study is to estimate the effect of occupational factors contributing to the incidence of lung cancer in an objective way. To estimate the effect of occupational factors to incidence of lung cancer, we used proportional incidence ratio(PIR) of each industry classification, population attributable risk(PAR) of each carcinogenic agents and the sum of PAR of each agents. Using Busan occupational lung cancer surveillance data, we obtained job history data and exposure data to carcinogenic materials of lung cacer patients in Busan area. We calculate PIR of each industrial calssification by industry classification of patients based on their job and 2005 census data. We estimate the number of population exposed to each carcinogens using cancer exposure index(CAREX index), turnover factor(TO) and 2005 census data in Busan. and then we calculate PAR and combine PARs for exposure agents. The PIR of agriculture, mining, transportation, construction and manufacturing was higher with statistical significance comparing with lung cancer incidence in the general population in Busan. The range of PAR of cadmium, asbestos, nickel, PAH, radon, chromium, silica was -3.11% to 7.52%, the sum of these was 9.17%. In this study, by estimating PIR, PAR of each agents and sum of PAR of lung cancer in Busan, we reveal high risk industries and the effect of occupational factors to incidence of lung cacer in total population. This study is the first step for prevention occupational lung cancer.
