This study was performed to investigate the effects of ground walking training(control group), stair and ramp gait training among gait training programs on the improvement of a static balance and gait in patients with stroke. The subjects of this study were 31 stroke patients and randomly assigned to a ground walking training group(n=10), or a ramp gait training group(n=10), or a stair gait training group(n=11). Each group performed the gait training in addition to a conventional neurological physical therapy five times per week for six weeks. In the pre and post experiment, the static balance(distribution for weight bearing of foot print, surface area ellipse of romberg, length of romberg, average speed of romberg, length/area of romberg, and limit of stability) was measured by AP1153 Biorescue, and the temporo-spatial parameters(step length, H-H base of support, step time, double support ratio, and gait velocity) were measured by RS-Scan system. The statistical analysis used One-way ANOVA to compare the difference among three groups. The results were as follows ; 1. In the change of distribution for weight bearing of foot print on the affected side and the non-affected side between pre and post value, there was no significant difference in the control group(p>.05), but there was a significant difference in the ramp gait training group(p<.05). There was a signigicant difference in the affected side of the stair gait training group(p.05). 2. In the change of surface area ellipse of romberg between pre and post value, there was no significant difference in the control group(p>.05), but there was a significant difference in the ramp gait training group and the stair training group. There was no significant difference the extent of change of surface area ellipse of romberg among three groups(p>.05). 3. In the change of length of romberg between pre and post value, there was no significant difference in the control group(p>.05), but there was a significant difference in both the ramp gait training group and the stair gait training group. There was no significant difference in the extent of change of length of romberg among three groups(p>.05) 4. In the change of average speed of romberg between pre and post value, there was no significant difference in the control group(p>.05), but there was a significant difference in both the ramp gait training group and the stair gait training group(p.05) 5. In the change of length/area of romberg between pre and post value, there was no significant difference in the control group(p>.05). There was a significant difference in the ramp gait training group(p.05). There was no significant difference in the extent of change of length/area of romberg among three groups.(p>.05) 6. In the change of anterior length in limit of stability between pre and post value, there was a significant difference in neither the affected side nor the non-affected side of the control group(p>.05). There was a significant difference in the affected side (p.05). There was no significant difference in the extent of change of anterior difference among three groups(p>.05). 7. In the change of posterior length in limit of stability between pre and post value, there was a significant difference in the control group(p>.05). There was a significant difference in the non-affected side (p.05). There was a significant difference in the affected side(p.05). There was a significant difference in the extent of change of posterior length in limit of stability among three groups(p>.05). 8. In the change of step length on the affected side and the non-affected side between pre and post value, there was a significant difference in neither the control group nor the ramp gait training group(p>.05), but there was a significant difference in the stair gait training(p.05). 9. In the change of H-H base of support on the affected side and the non-affected side between pre and post value, there was a significant difference in neither the control group nor the ramp gait training group(p>.05), but there was a significant difference in the stair training group(p.05). 10. In the change of step time on the affected side and the non-affected side between pre and post value, there was no significant difference in the control group(p>.05), but there was a significant difference in both the ramp gait training group and the stair gait training group(p.05) 11. In the change of double support ratio on the affected side and the non-affected side between pre and post value, there was a significant difference in neither the control group nor the ramp gait training group(p>.05), but the was a significant difference in the stair gait training group(p.05) 12. In the change of gait velocity between pre and post value, there was no significant difference in the control group(p>.05), there was a significant difference in both the ramp gait training group and the stair gait training group(p.05). The results of this study indicated that in comparison of pre and post changes of gait training, the stair gait training group has comparative changes than the other groups in the function of the static balance and gait. The stair gait training made a good progress of weight shift to the affected side during the stance phase by improving the stability of pelvis and trunk and the strength of the extensor muscles of the lower extremity, so it means that this training improved the ability of the static balance and gait. Therefore, the stair gait training would be more helpful to the improvement of the function of the static balance and gait than the other gait training methods.
