Objective: To collect and evaluate preceding studies about the effects of Oriental medicinal herbs and concoctions(OMHC) on blood coagulation and create a basis for further studies on the reaction of Oriental medicine on blood coagulation of patients with cerebral and cardiovascular diseases. Methods: An online-searching was performed and 204 oriental medicinal herbs and 58 types of oriental medicinal concoctions from 77 laboratory studies were classified into four groups by their anti-blood coagulation mechanism. Results: The OMHC acting similar to Heparin were Salviae Miltiorrhizae Radix(丹蔘), Daejo-hwan(大棗丸), and etc. The OMHC inhibiting the anticoagulant mechanism of heparin were Cimicifugae Rhizoma(升麻), Hyulboochooku-tang-gami(血府逐瘀湯加味), and etc. The OMHC acting similar to Warfarin were Persicae Semen(桃仁), Paeoniae Radix(白芍藥), and etc. while the OMHC inhibiting the anticoagulant mechanism of Warfarin were Cimicifugae Rhizoma(升麻), Dodamhwalhyul-tang(導痰活血湯), and etc. The OMHC acting similar to fibrinolytic agents were Scutellariae Radix(黃芩), Sopoong-tang(疏風湯), and etc. The OMHC inhibiting the anticoagulant mechanism of fibrinolytic agents were Rhei Rhizoma(大黃), Gwaehwa-san(槐花散), and etc. The OMHC acting similar to antiplatelet agents were Capreoli Cornu(鹿茸), Samul-tang(四物湯), and etc. The OMHC inhibiting the anticoagulant mechanism of antiplatelet agents were Carthami Flos(紅花), Gyukhachooku-tang(膈下逐瘀湯), Daehwangmokdanpi-tang(大黃牧丹皮湯), and etc. Magnoliae Cortex(厚朴), Hyulboochooku-tang(血府逐瘀湯), Chungpaesagan-tang(淸肺瀉肝湯), and Chunghyul-dan(淸血丹) increased the antiplatelet aggregation of Aspirin when used together. Conclusion: Based on these results, further study on the effects of OMHC on blood coagulation must take place in order to standardize a clinical criteria for using OMHC along with Western anticoagulant medicine on patients with cardiovascular or cerebrovascular diseases. This study may also act as a key to finding alternative treatments for patients who are in need of blood anticoagulation treatment but are unable to receive such due to side effects of current western medication or other issues. Furthermore, as some study results collide with others on the effects certain OMHC display on the blood coagulation system, the need for a standardized study model on this subject is emphasized.
