Genetically engineered stem cells (GESTECs) may be advantageous for gene therapy for treatment of various human cancers, including brain, hepatocarcinoma, ovary, and prostate. In this study, I used engineered human neural stem cells (HB1.F3.CD and HB1.F3.CD.IFN-cells) to express Escherichia coli cytosine deaminase (CD) and human interferon-beta (IFN-) genes as a therapeutic tool for breast cancer. A CD gene as a suicide gene can convert a non-toxic prodrug, 5-fluorocytosine (5-FC), to its active form, 5-fluorouracil (5-FU), which can induce a tumor-killing effect through DNA synthesis inhibition. IFN- also strongly inhibits tumor growth by the apoptotic process. Reverse transcription-polymerase chain reaction confirmed that HB1.F3.CD cells expressed the CD gene and HB1.F3.CD.IFN-cells expressed the CD plus IFN- genes. A modified Transwell migration assay showed that HB1.F3.CD and HB1.F3.CD.IFN-cells selectively migrated toward MCF-7 and MDA-MB-231 breast cancer cells. In co-cultured system to identify the therapeutic activity of the GESTECs, the viability of breast cancer cells was significantly reduced by HB1.F3.CD or HB1.F3.CD.IFN- cells in the presence of a prodrug, 5-FC. Especially, the inhibitory effect was much increased by HB1.F3.CD.IFN-cells, indicating the synergistic effect of 5-FU and IFN-. In addition, these GESTECs were identified to have tumor-tropic property by migrating to cancer cells, which can be attributed to the chemoattractant molecules such as stem cell factor, c-kit, CXCR4, vascular endothelial growth factor (VEGF), and VEGF receptor 2 secreted by breast cancer cells. An in vivo assay using breast cancer xenografts of mice implanted with MDA-MB-231/luc cells expressing a luciferase gene as a reporter gene demonstrated that these GESTECs effectively positioned at tumor sites, reduced tumor growth by 50% and prolonged the lifespan of mice. My results suggest GESTECs expressing CD and/or IFN-genes can be used as a novel targeted cancer gene therapy with a synergistically therapeutic potential against breast cancer in vitro and in vivo via their selective tumor tropism.
