The purpose of this study is to describe the injury mechanisms of ACL injuries in Alpine Skiing based on analysis of video injury situations. Seven specialists participate in the study: an orthopedic surgeon specialized in ACL reconstruction, a biomechanist specialized in ACL mechanism and ACL injury prevention, five imagery analysts(former alpine ski national team players with 2~6 times of ACL reconstruction experiences). The subjects of the study are three racing skiers and two recreational skiers. The specialists analyzed the video for five times based on analysis sheets with six levels of ACL injuries depending on the extent of injuries. For quantitative measures, intraclass correlation coefficient(ICC) is the principal measurement of reliability. In this study, ICC is estimated with Window version SPSS 18.0 analytic program. From the video analysis and reliability analysis, two main injury mechanisms for ACL are identified: sliding & grip and un-balanced landing. The first injury mechanism, a sliding & grip mechanism, occurs with a forceful valgus-internal rotation with the outer knee in great tension when the inside ski-edge grips the snow surface while a skier loses balance and sliding. The second injury mechanism, an un-balanced landing mechanism, occurs with forward movement of the tibia on femur(anterior drawer of the tibia) and excessive strain on knees when a skier lands improperly from a jump. Usually, un-balanced landing leads to land on the ski tails with extended knees, and the impact of snow surface is transmitted to knees through the skis. The transmitted impact causes anterior drawer of the tibia on femur and excessive strain on knees. As a result, the ACL injury may occur. In conclusion, noncontact ACL injury in Alpine Skiing may happen when a skier is losing balance with unreleased ski-bindings. The main injury mechanisms for ACL injuries in Apine Skiing are and valgus-internal rotation with the knee in great tension and anterior drawer of the tibia on femur.
