Background: The pandemic (H1N1) 2009 gives emphasis to the importance of enhancing the capability of existing influenza surveillance systems with tools for rapidly identifying novel, emerging viruses, while routinely surveying for seasonal influenza strains. Real-time reverse transcription polymerase chain reaction (RT-PCR) is widely considered the gold standard method for the molecular detection of influenza viruses due to the high assay specificity, sensitivity and broad linear dynamic range. In this study, we detected the pandemic (H1N1) 2009 virus using multiplex RT-PCR and two real-time RT-PCR assays and compared these methods. Patients and Methods: Nose and throat swab samples from 48 patients with influenza-like symptoms were tested using three assays. Multiplex RT-PCR and two real-time RT-PCR assays (Roche Molecular Diagnostics and Qiagen one-step RT-PCR kit) were evaluated on the same clinical samples. Discrepant results were confirmed by sequencing. Results: The sensitivity of multiplex RT-PCR was 62.5%. Among 13 discrepant samples, 12 were concordant with real-time RT-PCR results, only 1 sample had ambiguous result. The two real-time RT-PCR assays showed same results except one sample. Conclusion: The results of this study demonstrated that concurrent real-time RT-PCR assays could be used as primary diagnostic and confirmatory assays, and could provide rapid and accurate assessments of the novel pandemic (H1N1) 2009 strain. Multiplex RT-PCR assay could detect large number of samples at a time, laboratory services using multiplex RT-PCR should not exclude the possibility of false-positive results.
