是動病 所生病에 대한 硏究 : 開闔樞, 標本中 理論을 中心으로 (A) study on Shidongbing-Suoshenbing : the theories of the opening and closing pivot and the branch, root and centre are at the core원문보기

Shidongbing-Suoshenbing(是動病所生病) can be regarded as an essential component of studying both the twelve meridians and the process of development of acupuncture. Evidence for the significance of this study originates in references in major Oriental medicine textbooks including the Mawangdui scrolls(馬王堆帛書), , and . However, due to a lack of scholarly studies, it is difficult to use these references in a clinical context. Shidongbing-Suoshenbing in the Mawangdui scrolls are described as having a similar system to the differentiation of syndromes according to the Theory of the Six Meridians(六經辨證) in the Shanghanlun(傷寒論) On Cold Damage. Instead of the twelve meridians being changed to the Six Meridians, the concept became known as The Six Qi, this being due to the introduction of the concept of five elements from the . Owing to this, Shidongbing-Suoshenbing were considered as a kind of ZangFu(臟腑辨證) rather than Six Qi, causing much misunderstanding and many errors. This thesis analyzes Shidongbing-Suoshenbing on the basis of Kai-he-shu(開闔樞) Opening and Closing Pivot theory and Biao-ben-zhong(標本中) branch, root and centre. This thesis also advocates the Kai-he-shu physical constitution theory(開闔樞體質論) and the Jingxueluoxue-acupuncture point method(經穴絡穴鍼法) for clinical use. The first point to consider is that the eleven meridians of the Mawangdui scrolls have a vertical structure that connects the upper and lower parts of the human body like a universe in microcosm by means of the Six Qi(六氣). The meridians in the arm correspond to the ten Heavenly Stems(天干), while the meridians in the legs correspond to the twelve earthly branches(地支). In other words, ‘the root(根) and the knot(結)’ of can be said to mean the starting and ending points of the six leg meridians. Moreover, the arm meridians play a supportive role for the leg meridians. The arm meridians were not mentioned in the Kai-he-shu theory or ‘the Root and the Knot theory’ of because most of physio-pathological phenomena were recognized as being due to malfunctions in the six leg meridians. ZhuGong(朱肱) also defined the six Meridians(六經) of ShangHanLun as leg meridians. Kai-he-shu theory explained the physio-pathological phenomena of the six Meridians before the introduction of five phase theory(五行相生相剋論). Biao-ben-zhong(標本中)’s transformation theory(從化) describes the process in which the disease symptoms of Kai-he-shu manifest. It works as a method to differentiate a root illness(本病) from a branch(標病). It can be used to differentiate actual illness symptoms from false symptoms. Therefore, with the appearance of a symptom, the cause of the illness may be found by following Kai-he-shu theory. The treatment strategy may be decided after a doctor determines whether something is real or not according to Biao-ben-zhong(標本中) theory. Understanding the six Qi(六氣) is very important in understanding the physio-pathology of meridians. Yin-Yang(陰陽) of the six Qi implies simply more or less the quantity of Qi. It does not mean the differentiation of Qi on the basis of heat and cold. Actually, some people such as LiuWenShu(劉溫舒) insisted that the appearance and the essence of taiyang(太陽) and shaoyin(少陰) are different Qis. This can be attributed to the misunderstanding of yin-yang of the six Qi as heat and cold as discussed in the Theory of Four Constitutiions Sa Sang. Of the six Qi Jueyin(厥陰), has a unique double-sided quality of both yin and yang. In this case, yin that gathers appears as formless wind(風), simultaneously as formed yang manifesting as wood(木). Disturbances occurring in the yin yang of the four seasons in Kai-he-shu, may be explained with the help of modern understanding of the climate. If we categorize Korean people’s body constitutions according to understanding the climate, we may differentiate them into southerners and northerners. Southerners are known to be generally able to withstand heat and humidity but are liable to suffer from cold and damp pathogens according to their body constitution in Kai-he-shu theory. On the contrary, northerners are considered to be able to withstand cold and dry weather but are susceptible to heat and fire pathogens. Further, if the pulse appears as Shu(樞) slow, we can simply recognize the person as a southerner and vice-versa for northerners, this in accordance with Louis’s pulse theory. The Kai-he-shu body constitution can also be ascertained for a mother during pregnancy. If a pregnant mother generally has a lot of heat, prefers the cold, and has a good appetite, she could be considered to have a northern child. This mother may be prescribed Jingui Danggui Powder(金匱當歸散) to clear the blood and reduce heat. In contrast, if a pregnant mother feels sluggish, suffers from profuse perspiration, and has a poor appetite, she may give birth to a southerner child. This mother may be prescribed Jiawei Bazhentang(加味八珍湯 : Modified Eight Treasures Powder) reduce heat and to replenish energy. In this way, to manage the health of a baby during gestation can help the baby develop a strong constitution against minor illnesses by regulating the balance of Kai-he-shu. The view of Shidongbing-Suoshenbing has seen some minor changes in the process of development of Oriental medicine. The most significant change has been regarding the lung meridian(手太陰肺經). A totally new description of lung illness has arisen, completely separating the Lung from the juyang meridian(巨陽眽) and putting it together with taiyang illness symptoms(太陽證) as in the ShangHanLun. Further, arm symptoms, that is, those of the head and neck were omitted from the juyang meridian(巨陽眽). Also, the yangming meridian(陽明眽) and the shaoyang meridian(少陽眽) in the leg were re-categorized into the small intestine meridian(SI), the large intestine meridian(LI), and the triple burner meridian(TB) respectively. Shidongbing Suoshenbing in the shaoyin meridian(少陰眽) also included the various symptoms of heart and kidney diseases such as flying into a temper(善怒), heart-sinking(心惕), and being afraid of getting caught(如人將捕之). In this point of view, illness symptoms of the meridians in the arm can be cured with meridians in the leg with the Jingxueluoxue-acupuncture point method(經穴絡穴鍼法). The theory of Kai-he-shu holds that the large intestine governs fluids(大腸主津液), the small intestine governs humors(小腸主液), the bladder governs the sinews (膀胱主筋), the gallbladder governs the bones (膽主骨) and the stomach governs blood(胃主血). This involves Suoshenbing(所生病). Regulation of the body temperature with regards to maintenance of homeostasis in the human body involves the relationship of defense Qi (衛氣) with nutritive Qi (營氣), and is related to the bladder’s role in governing the sinews. The fluid flowing (津液) from the large intestine is a kind of fluid that is thin or thick as saliva, interstitial fluids of the trachea, or synovial fluids according to the body condition, as well as humor-type fluids (液) such as cerebrospinal fluid, synovial fluids, amniotic fluids and intracellular fluid. Aside from this, Kai-he-shu theory has a lot in common with Western medical theories. As shown above, the study of Shidongbing-Suoshenbing was originally developed from the point of view of six Qi and Kai-he-shu. However, it was reinterpreted and distorted on the basis of five phase theory, leading to contemporary treatments of disease of the six Qi tending to use WuShuXu(五兪穴) in the WuXing-acupuncture point method(五行鍼法). This thesis argues that it is more correct to treat disease of the six Qi with the way of Yuanxueluoxue-acupuncture point method(原穴絡穴鍼法) and Jingxueluoxue-acupuncture point method(經穴絡穴鍼法). In formulating prescriptions the prescriber should recognize that the same medicine may be used for different disease and using different theories. With this in mind, practitioners of acupuncture and Shanghanlun(傷寒論) should be well-informed on these two acupuncture methods for their value as the foundation of all kinds of Oriental medical acupuncture methods.
