Kinetic statistics were analyzed with the fifteen hemiplegic patients who have been undergoing neural physical therapy, walking at their most comfortable speed with mono-cane, quad-cane, and hemi-walker, which were set at 1 inch above greater trochanter, 2 inches above greater trochanter, greater trochanter, 1 inch below greater trochanter, 2 inches below greater trochanter, and distal wrist crease, respectively. One-way repeated measures ANOVA was performed to compare walking variables according to walking aid length, two-ways repeated measures ANOVA with between-subjects factor by functional state was conducted to analyze variables according to walking aid length and type. Multiple regression analysis was executed to establish the equation for walking aid length by considering physical characteristics. The results were that stride length of the affected side at 2 inches above greater trochanter was longer than those of both at greater trochanter and distal wrist crease, velocity at 2 inches above greater trochanter was faster than that of at greater trochanter. Peak contact pressure on the metatarsal, the mid-foot, and the lateral heel area of unaffected foot at above greater trochanter were greater than those of both at greater trochanter and distal wrist crease. Reference voluntary contraction on the unaffected side’s triceps brachii at distal wrist crease was the greatest among six different lengths, as seemed increasing fatigue. There was no significant difference on the kinetic values by both walking aid length and type according to current functional state. To sum up, the ideal walking aid length to minimize abnormal walking pattern and maximize normal gait is not at traditionally recommended greater trochanter but at 2 inches above greater trochanter, the regression equation which was acquired by considering hemiplegia's physical characteristics was as follows. Walking aid length (cm) = 30.939 + [0.514 × arm length (cm)] Therefore, if many studies regarding various function for adult hemiplegia also verify the compatibility at 2 inches above greater trochanter, the best specialized and individualized walking aid length will be adjusted only for the adult hemiplegia.
