국문요약 연구배경: 폐쇄성 수면무호흡증은 수면 중 기도폐쇄로 인하여 발생하는 간헐적 저산소증 및 수면 장애를 특징으로 하는 질환이다. 간헐적 저산소증은 산화 스트레스 및 염증을 유발함으로써, 폐쇄성 수면무호흡증과 연관된 다양한 합병증을 초래한다. 본 연구를 통하여 폐쇄성 수면무호흡에 만성적으로 노출된 토끼모델에서 여러 장기에 발생하는 합병증을 알아보고자 하였다. 대상 및 연구방법: 열두 마리의 New Zealand White rabbit이 실험에 이용되었다. 폐쇄성 수면무호흡은 연구개 및 혀의 기저부에 액화 silicone을 내시경 하에 주입하여 유도하였다. 무호흡유발 후1, 6, 12개월에 수면다원검사를 실시하였다. 무호흡 유발 후6 개월 (무호흡 유발군 토끼3마리, 대조군 토끼3마리), 12개월 (무호흡 유발군 토끼3마리, 대조군 토끼3마리)에 토끼를 희생하여, 각각 혀, 소뇌, 시상, 전두엽, 심장, 간, 경동맥, 대퇴동맥, 그리고 혈액을 채취하였다. 면역조직화학검사와Comet assay를 이용하여, 다양한 장기에서nitric oxide synthase (NOS), malondialdehyde, TNF-α, IL-10, ssDNA등의 생물지표를 조사하여 서로 비교하였다. 결과: 무호흡군에서 무호흡 유발 후 1, 6, 12개월의 무호흡-저호흡 지수가 대조군과 비교하여 유의하게 증가되었다 (p<0.05). 면역조직화학검사상 전두엽 및 심장에서 neuronal NOS, 소뇌에서 TNF-α, 그리고 소뇌 및 경동맥에서 IL-10의 발현이 대조군과 비교하여 유의한 차이가 나는 것이 관찰되었다. 심장은 대조군과 무호흡군간의 유의한 차이가 무호흡유발 후 6개월에 관찰되었고, 전두엽, 소뇌, 경동맥은 12 개월에 관찰되었다. Therefore, the aim of this study is to investigate complications of chronic exposure to OSA in rabbit models. Materials and Methods: Twelve New Zealand White rabbits were used for the experiment. OSA was induced by injection of liquid silicone to the soft palate and base of tongue under the endoscopic guidance. Polysomnography was performed at 1, 6, and 12 months after induction of OSA. Rabbits were sacrificed at 6 months (3 rabbits with OSA and 3 control rabbits) and 12 months (3 rabbits with OSA and 3 control rabbits) after induction of OSA, and the tongue, cerebellum, thalamus, frontal lobe, heart, liver, carotid artery, femoral artery, and blood sample were taken for specimens. A diversity of biomarkers for IH and inflammation, such as nitric oxide synthase (NOS), malondialdehyde, TNF-α, IL-10, and ssDNA, were evaluated in the multiple organs by means of immunohistochemistry (IHC) and comet assay. Results: Apnea-hypopnea index was significantly increased in the OSA group compared to the control group at 1, 6, and 12 months after induction (p<0.05). In IHC, a significant difference between the OSA and control group was observed in neuronal NOS expression of the frontal lobe and heart, TNF-α of the cerebellum, and IL-10 of the cerebellum and carotid artery. In the heart, a significant difference between the OSA and control group was observed at 6 months after induction, but in the frontal lobe, cerebellum, and carotid artery, at 12 months. In comet assay, there was a significant difference in length of the comet tail per length of the comet head (CT/CH) between the OSA and control group in the cerebellum and thalamus (p<0.05). In the cerebellum, CT/CH in OSA and control group was 6.78±1.45 and 1.41±0.26, and in the thalamus, 2.79.±0.79 and 0.85±0.24 respectively.
