Acquired valvular heart disease is the major cause of coughing, dyspnea, syncope, pulmonary edema, pulmonary hypertension, and sudden death in geriatric dogs. The purpose of this study was to compare the prevalence and characteristics of physical examinations, hematologic, radiographic, electro-cardiographic, and echocardiographic features of dogs with acquired valvular diseases among survivors and non-survivors; to analyze pre- and post-treatment effect; and to estimate the survival times of acquired valvular heart disease patient with respected to some new prognostic factor.
Small to middle-sized dogs (n=170) with acquired valvular heart disease treated at the Veterinary Teaching Hospital of Konkuk University from March 2009 to June 2012 were included. One hundred twenty eight dogs had mitral valve insufficiency, 3 dogs had tricuspid valve insufficiency, and 39 dogs had bivalvular insufficiency. All the dogs with acquired valvular disease were classified into survival (S) and non-survival (NS) group. The NS group was subdivided into less-than-one-year-survival (1 year). To evaluate treatment effects, case records of 64 dogs that had undergone re-evaluation were selected from the 170 dogs and classified into the S and NS groups. Treatment variables, complicated disease, and reason of death were also determined.
Prevalence characteristic of this study, castrated males (41.8%), Maltese (35.5%) and 11 to 15 year-old dog (51.5%) were dominant. High grade murmur, ISACHC class, dyspnea, and activity affected survival time. The type of valvular insufficiency was also a prognostic factor.
Blood test results showed that complete blood cell count and electrolytes did not affect survival time, but some serum biochemistry profiles including blood urea nitrogen, aspartate aminotransferase, albumin, and phosphorus were prognostic factors. Radiographic presence of pulmonary edema and severity of cardiac remodeling affected survival time. On echocardiographic evaluation, the type of the affected valve, severity of mitral regurgitation, and fractional shortening were prognostic factors.
In evaluation of treatment effect groups, coughing, dyspnea, activity, syncope, and white blood cell count were associated with improvement in the S group. However, in the NS group, only improvement of coughing and increasing pulmonary pressure gradient was evaluated.
In conclusion, to predict survival time and prognosis of the dogs with acquired valvular heart disease, ISACHC class, murmur grade, dyspnea, activity, type of valvular insufficiency, increased plasma blood urea nitrogen, aspartate aminotransferase, phosphorus, low plasma albumin level, presence of pulmonary edema/hemorrhage, severity of cardiac remodeling with mitral regurgitation, and fractional shortening need to be evaluated. Evaluation for improvement of these clinical symptoms after treatment and owner education concerning steady medication can be useful in improving survival times and in predicting prognosis.
