본 연구에서는 각막 이심률에 따른 RGP 렌즈의 중심안정위치 및 동적 움직임의 상관관계를 알아보고자 하였다. 각막 이심률이 0.28~0.78인 각막 84안을 대상으로 구면 및 비구면 RGP 렌즈의 피팅 상태를 달리하여 착용시키고 각막에서의 중심안정위치를 초고속 카메라로 촬영하고 photoshop 프로그램을 사용하여 분석하였다. 구면과 비구면 RGP 렌즈의 수평방향 중심안정위치는 이심률이 작은 각막에서는 귀쪽으로의 치우쳐짐이 컸으나 이심률이 큰 경우는 각막 중심에 더 가까이 위치하였다. 구면 및 비구면 RGP 렌즈 모두 피팅 상태가 플랫할수록 이심률에 따른 중심안정위치의 차이가 커졌으며, 비구면 RGP 렌즈의 경우 각막 이심률이 작은 경우와 큰 경우 모두 중심안정위치의 차이가 구면 RGP 렌즈 보다 더 적었다. 구면 및 비구면 RGP 렌즈 착용 시 수직방향으로의 중심안정위치는 피팅 상태에 상관없이 모두 각막 중심을 기준으로 아래 쪽에 위치하였으며 각막 이심률에 따른 차이는 통계적으로 유의하지 않았다. 그러나 이심률이 클수록 RGP 렌즈가 상안검에 맞닿는 경우가 많았다. 구면과 비구면 RGP 렌즈는 양호한 피팅 상태에서 각막 이심률이 커질수록 통계적으로 유의하게 하방이동량이 감소하였으며, 렌즈 디자인에 따른 차이는 없었다. 구면 렌즈의 경우 스팁한 피팅과 양호한 피팅에서, 비구면 렌즈는 양호한 피팅과 플랫한 피팅에서 각막 이심률과 하방이동량의 상관관계가 통계적으로 유의하게 감소하였다. 이심률이 증가함에 따라 안검폭의 수직길이가 감소하는 것으로 나타나 이심률위 증가에 따른 하방이동량의 변화추이는 각막 만곡도의 차이뿐만 아니라 안검폭의 길이에 의해서도 영향을 받는 것을 알 수 있었다. 하방이동속력의 경우 일반적으로 알려져 있는 바와 같이 구면과 비구면 렌즈에서 스팁, 양호, 플랫한 피팅 순으로 하방이동속력이 느려졌으며 통계적으로 유의한 차이가 있었다. 또한 구면 렌즈에서 각 피팅 상태에서 모두 비구면 렌즈보다 더 빠른 하방이동속력을 나타냈다. 이심률과의 상관관계에서 는 구면 및 비구면 RGP 렌즈 모두에서 이심률의 변화에 따른 상관관계가 컸으며 스팁한 피팅이나 플랫한 피팅의 경우 상관성이 떨어졌다. 하방이동속력에서도 양호한 피팅에서 그래프의 추세선 기울기가 컸으나 통계적으로 유의한 차이는 없었다. 회전움직임을 보면 양호한 피팅시에는 구면 렌즈의 경우 거의 모든 이심률에서 회전움직임이 거의 없었으며, 비구면 렌즈에서는 각막 이심룰이 증가할수록 6시 방향을 기준으로 귀쪽으로 회전하는 정도가 커졌다. 구면 렌즈에서 회전이 크게 일어나는 경우는 플랫한 피팅시이며 각막 이심률이 작은 경우에는 귀쪽으로, 이심률이 클 때는 코쪽으로 회전하는 경향을 보였다. 구면 렌즈의 경우 일반적으로 플랫한 피팅에서 정점회전이 잘 일어난다고 알려져 있는것과 동일한 결과를 보였다. 그러나 비구면 렌즈의 경우 구면 렌즈와 달리 각막 이심률이 큰 경우 양호한 피팅 상태에서 회전정도가 더 컸으며, 오히려 플랫한 피팅에서는 회전정도가 작아 비구면 렌즈에서는 일반적인 정점회전패턴양상이 적용되지 않음을 알 수 있었다. 본 연구 결과 각막 이심률에 따라 구면과 비구면 RGP 렌즈의 중심안정위치와 하방이동속력 하방이동량, 회전정도가 다르며 피팅 상태에 따라서도 차이가 있음을 알 수 있었다. 각기 다른 이심률을 가진 다양한 각막에는 적용되지 않는 어려움이 있어 RGP 렌즈의 피팅 및 제조 시에 각막 이심률을 고려하여 좀 더 다양한 요인들을 고려하여야 할 것으로 보인다.본 연구에서는 각막 형상 및 난시도가 상이한 각막에 구면 및 비구면 RGP 렌즈를 피팅 상태를 달리하여 착용하였을 때 각막과 렌즈와의 간극에 미치는 영향을 알아보아 중심부와 주변부의 플루레신 염색 면적이 각막 형상 및 난시도에 따라 어떻게 달라지는 지를 비교하여 렌즈와 각막과의 정렬상태가 렌즈의 디자인에 따라 어떠한 차이가 있는지를 알아보고 적절한 RGP 렌즈의 선택에 도움이 되고자 하였다. 본 연구의 취지에 동의한 사람 가운데 안질환이나 사시, 약시, 굴절이상 교정수술 등 안과적 수술경험이 없고, 콘택트렌즈 착용 경험이 없으며, 복용하는 약물이 없는 19~30세(평균 25.12±3.52세)의 성인 남, 여 252안을 대상으로 하였다. 각막지형도 검사로 대상자의 각막 형상을 원형(round), 타원형(oval), 대칭나비형(symmetric bowtie), 비대칭나비형(asymmetric bow tie), 부정형(irregular)의 5가지로 분류하였으며, 그 중 원형과 대칭나비형 각막 90안을 최종 연구대상자로 선정하였다. 수동 각막곡률계를 사용하여 대상자의 각막곡률을 3회 측정한 후 각각의 각막형태별로 난시량에 따라 다시 세분화하여, 피검안을 총 5개의 군으로 나누었다. 세극등 현미경의 코발트 블루 조명 하에서 플루레신 용지로 각막 표면을 염색하여 렌즈가 각막 위에 위치한 상태를 나타내는 정적 피팅 평가를 시행하였다. 렌즈의 베이스커브를 조절하여 ± 0.05 ㎜ 단위로 구면 및 비구면 RGP 렌즈를 피팅 상태를 달리하여 착용시키고 플루레신 염색 패턴을 분석하였으며, 이의 정량적인 평가를 위하여 중심부와 중간주변부 및 주변부의 면적을 계산하여 각막과 콘택트렌즈의 간극을 비교하였다. 구면 RGP 렌즈와 비구면 RGP 렌즈가 양호한 피팅이 되는 렌즈의 베이스커브가 두 렌즈에서 착용시 동일한 경우는 72%에 달하였으나, 대칭나비형 각막과 높은 난시도를 가진 경우에는 동일한 베이스커브에서 구면 RGP 렌즈와 비구면 RGP 렌즈가 양호한 피팅이 될 가능성이 적어졌다. 양호하게 피팅된 비구면 RGP 렌즈의 주변부 플루레신 염색 면적은 구면 RGP 렌즈보다 더 작았으며, 두 디자인의 렌즈 모두 난시가 심해질수록 주변부 면적은 감소하였고 원형 각막보다도 대칭 나비형 각막에서의 주변부 면적이 더 작았다. 동일한 난시도의 각막에서는 두 각막 형상 모두에서 양호하게 피팅된 비구면 RGP 렌즈의 주변부 플루레신 염색 면적이 더 작았다. 동일한 난시도인 0.75~1.25 D에서 각막 형상에 따라 RGP 렌즈가 피팅되었을 때 원형 각막의 경우 구면과 비구면 렌즈의 베이스커브 일치도가 92%로 높은 반면, 대칭나비형 각막에서는 65%의 일치도를 보여 각막 형상에 따라 RGP 렌즈의 피팅에 차이가 있었다. 따라서 현재 사용되고 있는 난시도만을 고려한 RGP 렌즈의 가이드 라인은 충분하지 않다고 사료된다. 각막 형상에 따라 플루레신 패턴으로 중심부와 주변부 면적을 분석하였을 때 양호한 피팅에서는 두 렌즈 디자인 모두에서 중심부와 주변부 면적이 각막 형상에 따라 통계적으로 유의하게 달라졌으나, 스팁하게 피팅되었을 때 중심부 면적은 각막 형상과 렌즈 디자인에 따른 차이가 없었다. 플랫 피팅의 경우 렌즈 디자인에 따른 차이는 없었으나 두 렌즈 디자인에서 원형 각막의 중심부 면적이 더 커 눈물 순환 및 렌즈의 움직임이 렌즈의 피팅 상태와 각막 형상에 따라 달라질 수 있음을 유추해 볼 수 있다. 중간주변부는 스팁하게 피팅되었을 경우 나타났는데, 렌즈 디자인이나 각막 형상에 따른 차이는 없었다. 주변부 면적은 스팁한 피팅시 렌즈 디자인에 따른 차이를 보였고, 각막 형상에 따른 비교에서는 두 형태 모두 구면 렌즈에서 주변부 면적 비율이 높았다. 즉 비구면 렌즈를 착용시 주변부 폭의 면적 감소로 원형각막에서의 눈물 순환이 상대적으로 더 감소할 수 있음을 알 수 있었다. 플랫한 피팅에서는 주변부 면적은 렌즈 디자인에 따른 차이는 없었으나 각막 형상에 따른 차이를 보였다. 각막 난시도에 따라 플루레신 패턴의 면적을 분석하였을 때, 양호한 피팅의 경우는 중심부와 주변부 면적이 각막 형상에 상관없이 각막난시가 증가하면 일정하게 증가 혹은 감소하는 양상을 보였으나, 스팁 피팅과 플랫 피팅의 경우는 원형 각막과 대칭나비형 각막에서의 면적 변화 양상이 상이하였다. 본 연구를 통하여 RGP 렌즈의 각막과의 정렬은 렌즈 디자인, 각막의 난시도, 각막 형상에 의해 차이가 있음을 알 수 있었다. 스팁이나 플랫하게 피팅 되었을 경우 양호한 피팅시 보다 각막난시나 각막 형상에 따른 렌즈와 각막과의 간극 변화가 더 커지며 이러한 결과는 구면 RGP 렌즈와 비구면 RGP 렌즈 모두에서 공통적으로 나타나는 것임을 알 수 있었다. 따라서 렌즈 디자인과 각막의 정렬상태의 정량적인 고찰을 통한 본 연구결과는 RGP 렌즈의 피팅 가이드라인 설정, 적합한 디자인 개발, 부분별 누액양에 대한 기초자료로 사용될 수 있을 것이다.

The present study was performed to investigate a relationship between the stable centrations of spherical RGP lens and aspherical RGP lens on cornea and corneal eccentricity. Two RGP lenses with different designs were fitted in alignment, steep or flat on total 84 eyes having corneal eccentricity of 0.28~0.78. The stable centration of lenses on cornea was analyzed by taking photographs with a very high-speed digital camera. The stable centration of spherical and aspherical RGP lenses in horizontal direction was leaning toward to temporal side, but it was located closer to the center of cornea with larger corneal eccentricity. The difference between the stable centration of spherical and aspherical RGP lenses according to corneal eccentricity was larger when the fitting status was flatter. The difference of the stable centration of aspherical RGP lens was smaller than that of spherical RGP lens regardless of fitting status in case of small corneal eccentricity. The stable centration of both spherical and aspherical RGP lenses in vertical direction was located below corneal apex regardless of fitting status, and there was no significant difference analyzed by the variation of corneal eccentricity. However, RGP lens frequently touched the upper eyelid with increase of corneal eccentricity. When spherical and aspherical RGP lenses was in alignment fitting, the downward flow rate significantly decreased in statistic with corneal eccentricity increased, and there was no significant difference by lens design. When spherical lens was fitted in alignment and steep and aspherical lens was fitted in alignment and flat, the correlation between corneal eccentricity and downward flow rate was significantly was significant. The relationship between the stable centration and downward flow rate was in contrast, but this was considered because of different variables that effect on. Thus, it was confirmed that downward flow rate could be influenced by vertical height of palpebral fissure width. In case of downward flow speed, it was slower in order of fitting status as steep, alignment and flat as generally known and the difference was significant. The downward flow speed was faster in all spherical lenses other than aspherical lenses. In the correlation with eccentricity, it was significantly correlated to the change of eccentricity in both of lenses, but less correlated in case of steep and flat fitting. Also, the slope of trend line of the plot was steep in alignment fitting but there was no significant difference. There was almost no rotation when spherical lens was fitted in alignment in case of all eccentricity, and it was more rotated in direction to the ear from the direction of 6 o’clock with increase of corneal eccentricity. The rotation was significant when spherical lens was fitted in flat, and it tended to rotate toward to the ear with less eccentricity and to the nose with more eccentricity. Thus, the consideration of corneal eccentricity is required for RGP lens fitting and manufacturing aspherical RGP lens since it is not able to apply to various corneas with different eccentricities.In the present study, the effect of lens fitting status on the contact area between spherical and aspherical RGP lens and the cornea having different astigmatic degree and corneal type was investigated for guiding the proper selection of RGP lens. The 252 eyes of male and female candidates in an age range of 19 to 30 (average age of 25.12±3.52) without formal experience of contact lens, medication or any kind of ophthalmic surgeries for ophthalmic diseases, strabismus, amblyopia, or anomalies of refraction were selected from those who agreed with the purpose of the study. By corneal topography test, the corneal type of eyes was categorized as 5 different types: round, oval, symmetric bow tie, asymmetric bow tie and irregular type, and 90 final eyes with round and symmetric bow tie corneal type were selected from all eyes. After the 3 measurements of the corneal curvature of the eyes were taken by manual keratometry, the final eyes were sorted as 5 groups by subdividing according to astigmatic degree for each corneal type. The static fitting evaluation showing how the lens was fitted on cornea was performed by dying the surface of cornea with a fluorescein sheet under cobalt blue light of slit lamp microscope. The patterns of fluorescein dye were analyzed after the spherical and aspherical RGP lens was worn in the various ways of the fitting status of the lenses within a unit of ± 0.05 ㎜ by controlling base curve of lens, and the gaps between cornea and contact lens were analyzed for the quantitative evaluation based on the calculations of central, mid-peripheral and peripheral area. The case showing concordant base curve between spherical and aspherical RGP lenses in alignment fitting was up to 72% however, the possibility to have same base curves between spherical and aspherical RGP lenses in alignment fitting was to be less in the case of symmetry bowtie-typed cornea and high astigmatism. The fluorescent area in peripheral region of aspherical RGP lens in alignment fitting was smaller than it of spherical RGP lens. Peripheral fluorescent areas in both RGP lenses decreased according to the increase of astigmatic degree and peripheral area in symmetric bowtie-typed corea was smaller than round-typed cornea’s peripheral area. On the cornea with same astigmatic degree, peripheral fluorescent area of aspherical RGP lens was smaller in both corneal types. In the same astigmatic degree, 0.75 – 1.25 D, the concordant base curve between spherical and aspherical lenses was high up to 92% in the case of round-typed cornea in alignment fitting according to corneal type but only 65% of concordant base curve in symmetric bow tie-typed cornea, as a result, difference in fitness of RGP lens was significant according to corneal type. Thus, the current guide line of RGP lens considering only astigmatism was not satisfied for the proper selection of the RGP lens. The central and peripheral areas with the alignment fitting were significantly different based on lens design. However, the central area didn't show any significant difference by both lens designs and corneal type when fitted in steep. In case of flat fitting, the difference was not significant by lens design. However, it was interpreted that the circulation of tears and movements of lens could be varied by the fitting status or corneal type since the central area of the round-typed cornea in two lens designs was larger. In steep fitting, no significant difference was shown in mid-peripheral area by lens design or corneal type. The peripheral area showed a significant difference when fitted in steep, and the ratio of the peripheral area was high in both types in spherical lens when analyzed by corneal type. Therefore, it is known that the circulation of tears in round-typed cornea was relatively decreased with decrease of peripheral area while wearing aspherical lens. In the flat fitting, peripheral area had no difference for lens design but for corneal type. When analyzed by the astigmatic degree, the central and peripheral areas with alignment fitting changed proportionally with the increase of corneal astigmatism regardless of corneal shape. With steep and flat fitting, however, the central, mid-peripheral and/or peripheral areas in round- and symmetric bowtie-typed corneas showed the conflicting result when compared to those of alignment fitting when analyzed by the astigmatic degree. In this study, it was known that the fitting between RGP lens and cornea was significantly different by lens design, corneal astigmatism and corneal. The gap between cornea and RGP lens fitted steep and flat status was increased rather than fitted in alignment status. Also, this result commonly occurred in both spherical and aspherical RGP lenses. Therefore, this study will be considered as a reference in establishment of a guideline of RGP lens fitting, development of proper design and different volumes of tear in partial regions.
The present study was performed to investigate a relationship between the stable centrations of spherical RGP lens and aspherical RGP lens on cornea and corneal eccentricity. Two RGP lenses with different designs were fitted in alignment, steep or flat on total 84 eyes having corneal eccentricity of 0.28~0.78. The stable centration of lenses on cornea was analyzed by taking photographs with a very high-speed digital camera. The stable centration of spherical and aspherical RGP lenses in horizontal direction was leaning toward to temporal side, but it was located closer to the center of cornea with larger corneal eccentricity. The difference between the stable centration of spherical and aspherical RGP lenses according to corneal eccentricity was larger when the fitting status was flatter. The difference of the stable centration of aspherical RGP lens was smaller than that of spherical RGP lens regardless of fitting status in case of small corneal eccentricity. The stable centration of both spherical and aspherical RGP lenses in vertical direction was located below corneal apex regardless of fitting status, and there was no significant difference analyzed by the variation of corneal eccentricity. However, RGP lens frequently touched the upper eyelid with increase of corneal eccentricity. When spherical and aspherical RGP lenses was in alignment fitting, the downward flow rate significantly decreased in statistic with corneal eccentricity increased, and there was no significant difference by lens design. When spherical lens was fitted in alignment and steep and aspherical lens was fitted in alignment and flat, the correlation between corneal eccentricity and downward flow rate was significantly was significant. The relationship between the stable centration and downward flow rate was in contrast, but this was considered because of different variables that effect on.
