Purpose The purpose of this study was to examine changes in the prevalence and progression rate of myopia in groups of the same age over the course of 15 years. Method Manifest refraction was evaluated to identify any change in the average refractivity and prevalence of myopia in 6-year old children who were at the end of emmetropization. The subjects were 8,129 6-year old children (4224 boys, 3,905 girls) who had attended nursery schools in Daejeon between 1997 to 2012. In addition, in order to identify differences in the prevalence and progression rate of myopia in older age groups, a cross-sectional study was conducted on 5∼50 year-old individuals who had visited optical shops in Cheongju and Cheonan. There were 162,494 cross-sectional subjects in total (50,453 males and 112,041 females), and their right eyes were examined. An analysis of trends over time was conducted, and a total of 64,292 right eyes (15,557 male eyes, 48,725 female eyes) of individuals who visited an optical shop more than twice were tested. The progression rate of myopia was estimated and compared between different age groups. The effects of the year of their optical shop visit and the birth year on the myopia progression rate were statistically tested using a linear mixed model. To compare the generational differences (deemed to be the causes of changes in myopia) in near tasks, a questionnaire survey was performed with 117 subjects in their twenties (56 males, 61 females) and 84 subjects in their thirties (27 males, 57 females). Results Six-year old children showed a significant difference in their type of refractivity according to their date of birth, and the degree of average refractivity changed in the (-) direction in the emmetropic group, myopic group and astigmatic group in accordance with their date of birth. When the degrees of average refractivity were compared after categorizing the subjects into individuals born before and those born after a certain year, all groups but the hyperopic group showed a significant difference in the (-) direction. Children who were born later showed a stronger tendency in the (-) direction in the degree of their refractivity. Among 5-50 year old optical shop visitors, all age groups showed a high average refractivity in the (-) direction with the exception of the groups between age 5 and 9. The change in the degree of average refractivity was most significant when the subjects were categorized into groups based on age - pre-school, elementary school, middle school, high school and post-high school. Regarding the changes in the distribution of the types of refractivity by age in groups between 6-24 years old, the subjects showed gradual decreases in emmetropia and myopia. However, there was no significant change in the prevalence of hyperopia or astigmatism according to the year of optical shop visit. The results of an analysis of the changes in the distribution of the types of refractivity showed a decrease in emmetropia and an increase in myopia in all age groups. In the analysis of increased myopia in all age groups according to the year of optical shop visit, low myopia decreased, and moderate myopia and high myopia increased according to the year of optical shop visit. The same trend appeared in all age groups except the 6-7 year olds. High myopia, which was very rare in the early 1900s, started appearing within the 8-13 year old age group in the 2000s and the ratio of high myopia has continued to increase in the other age groups as well. A regression analysis to identify the effect of the year of optical shop visit on the average refractivity of children who attended nursery schools showed that degree of refractivity of those who visited optical shops later was likely to change in the (-) direction. The 8-13 year old group showed a significant difference in VDD(vector difference in diopter) values of subjects who visited the same optical shops two or more times, according to the year of visit. The VDD(vector difference in diopter) values of individuals who visited the same optical shop 5 or more times were significantly different in both the 8-13 year old group and 14-16 year old group. Changes in the degree of average refractivity of the 6∼24-year old group and the 6∼15-year group who visited the same optical shop two or more times, according to the year of visit and date of birth, were analyzed through a linear mixed model. The degree of average refractivity increased in the (-) direction according to the year of visit while gender and date of birth were controlled. In a questionnaire survey to compare the amount of near tasks performed according to birth year, those in their twenties spent more time learning and reading books as well as watching TV and gaming during their school-age years than those in their thirties. Conclusions The rapid changes in myopia prevalence and progression suggest the environmental factors have more influence than genetic factors. An increase in near tasks is the most important environmental factor varying by generation. A bigger problem than the prevalence of myopia itself is that myopia is qualitatively worsening in terms of prevalence rate and progression rate. Younger age groups start having myopia earlier and its prevalence is also increasing, which means the complications of severe myopia will also increase. Despite the significant increase in the prevalence of myopia, less attention is paid and the number of related studies is decreasing due to the availability of correction methods such as refractive surgery and glasses. However, more studies on myopia prevalence and progression and its secondary complications are still needed.
