People with diabilities use wheelchairs to cope with the restraints on movements. As their social participation increases, people with diabilities use wheelchairs for longer hours, which can cause a pressure ulcer. It is necessary for people with diabilities to use wheelchair seat cushions to prevent any pressure ulcer. The seat cushions provided for cerebral palsy as part of the aids supply project mostly require air-pumping. This study aims to provide reference data on wheelchair seat cushions by comparing pressure distribution in line with physical features on the air-pumping and other types of cushions mostly used by persons with cerebral palsy at home. This study made or purchased a polyurethane foam cushion, an inexpensive air cushion, a common air cushion and a latex cushion mostly used by those with cerebral palsy for the comparative analysis. Specifically, the pressure distribution was measured following a 5-minute sitting on each cushion. Then, the values of mean pressure, maximum pressure, total contact area and area ratio of more than 60mmHg of the total contact area were collected as per 2, 3, 4 and 5-minute frames. Next, the collected values were divided by body weights, hip widths, thigh lengths and body mass indexes to get mean values. Below are the findings of this study. First, the mean pressure was found lowest on the polyurethane foam and common air cushions. The maximum pressure was lowest on the inexpensive air and latex cushions. The total contact area was widest on the polyurethane foam and latex cushions. The area ratio of more than 60mmHg of the total contact area was found lowest on the inexpensive and common air cushions. These findings indicate that the polyurethane foam and latex cushions effectively disperse the pressure by widening the total contact area instead of reducing the pressure. Second, a person with disability or non-disability was found not to have significant effects on the seat pressure distribution. Nonetheless, the common air cushion showed differences in the maximum pressure and the area ratio of more than 60mmHg of the total contact area between the disability and non-disability in the normal body weight group(p<.05). Third, the degree of comfort felt on each cushion varied with disability or non-disability(p<.05). People without disabilities found the air-pumping inexpensive and common air cushions highly comfortable. By contrast, people with disabilities found the latex and polyurethane foam cushions highly comfortable. Fourth, the inexpensive air cushion showed the least increase in the mean pressure and the highest increase in the total contact area in line with the increase in body weights and body mass indexes(p<.05). The latex cushion proved the least increase in the area ratio of more than 60mmHg of the total contact area, corresponding to a high pressure, in line with the increase in body weights and body mass indexes(p<.05). Fifth, body weights and body mass indexes were found to influence the mean pressure, the total contact area and the high-pressure area ratio (p<.05). The higher the body mass indexes, the higher the mean pressure, the total contact area and the area ratio of 60mmHg of the total contact area. Sixth, hip breadth led to a less maximum pressure on the latex cushion and a wider total contact area on the polyurethane foam, inexpensive air, common air and latex cushions(p<.05). The longer the thigh, the wider the total contact area on the latex cushion(p<.05). Taken together, the inexpensive air cushion is as effective as the common air cushion priced as much as at 350,000 Won in terms of lessening the maximum pressure, which suggests that people with disabilities who are not beneficiaries of the assistive devices support program may use the inexpensive air cushion to prevent a pressure ulcer. Still, extra care need to be taken in using the PVC-based inexpensive air cushion, as it is fragile and contains chemical ingredients although it is non-toxic. Also, as the seat pressure increases in proportion to body weights and body mass indexes, and as the pressure increase due to obesity raises the risk of a pressure ulcer, people with disabilities need control their eating habits and take some exercise.
