Individual Brain Atlases using Statistical Parametric Mapping (IBASPM)을 이용하여 내측두엽 부피를 측정함으로써 임상에서 쉽게 알츠하이머병을 선별할 수 있도록 하고자 이 연구를 시행하였다. 알츠하이머병으로 진단된 94명의 환자와 51명의 정상인을 대상으로 하였다. T1-weighted magnetic resonance images (MRI)를 촬영한 후 IBASPM을 이용하여 양측 해마를 비롯한 내측두엽 구조물의 부피를 측정하였고, 환자군과 정상군을 서로 비교하였다. 알츠하이머병을 정상과 구분하는 부피의 기준치를 구하는데 연령별로 그룹을 나누어 시행하여, 나이에 따른 오차를 줄이고, 특정 나이에 따른 기준치를 구하였다. 결과적으로 정상화된 좌측 해마의 부피는 모든 연령군의 환자에서 정상에 비하여 유의하게 감소되어 있는 것을 확인할 수 있었다. 또한 수신자 조작 특성 곡선(Receiver operating characteristic curve, ROC curve)에서 좌측 해마부피의 곡선하면적(Area Under Curve, AUC)은 모든 연령군에서 0.7이상의 값을 보여 좌측 해마 부피가 알츠하이머병을 선별하는데 다른 내측두엽내의 구조물 보다도 유용하게 이용될 수 있음을 나타내었다. 알츠하이머병을 선별하는 좌측해마 부피의 기준치는 60-64세 연령 군에서 4.06, 65-69세는 4.05, 70-74세 연령 군에서는 4.07이었고, 마지막으로 75세 이상 연령 군에서는 3.63이었다. IBASPM을 이용한 좌측 해마부피의 측정은 기준치를 이용하여 임상에서도 쉽게 알츠하이머병을 선별해 내는데 유용하게 사용 될 수 있음을 증명하였다. 향후 대규모의 연구를 통하여 보다 보편적인 기준치를 마련하여 임상에서 사용할 수 있도록 하겠다.

We validated the use of medial temporal volume measurements via the Individual Brain Atlases in the Statistical Parametric Mapping (IBASPM), an automated volumetric method, for screening Alzheimer's disease (AD). We also determined the optimal cut-off values for hippocampal volumes. The study included T1-weighted magnetic resonance images (MRI) of 89 patients with AD and 33 normal controls. To determine age-specific cut-off values, we grouped subjects according to age. The regional structural volumes of each group were measured using individual atlas-based volumetry with IBASPM. The normalized value of left hippocampal volume in patients with AD was significantly lower than that of the normal controls in all age groups. The areas under the curve (AUC) values of the left hippocampus in ROC (receiver operating characteristic) curve were also the largest of regional structural volumes in medial temporal lobe, indicating that left hippocampal volume is the most useful factor of AD screening. The optimal cut-off values of left hippocampus for diagnosing AD were 4.06 in the 60-64 age group, 4.05 in the 65-69 group, 4.07 in the 70-74 age group and 3.63 in patient older than 75. Measurements of left hippocampal volume using the IBASPM may be useful for screening for AD in clinical practice, and could provide a useful biomarker for AD.
