In this thesis, a systematic radiation dose management program fit for the Korea's medical situation has been implemented based on the collected database of the individual patient's history and radiation dose on the digital radiography(DR) examinations in order to effectively reduce the patient's radiation dose for the good image-quality radiograph, and eventually to reduce the probability of cancer caused by the medical radiation dose. Film-based radiography has played an important role in the development of the radiography since the X-ray was discovered in 1895. Especially, various medical examinations with radiation have been done in the medical fields by securing the legitimacy for their actions. Now, the ICRP(International Commission of Radiation Protection) strongly recommends the medical centers to minimize the radiation doses to the patients for the high-quality medical images based on the concept of the ALARA (As Low As Reasonably Achievable). There have been big differences in the radiation doses exposed to the patients depending on the examined organs of the patients, medical centers and countries. According to the UNSCEAR (United Nations Scientific Committee on the Effects of Atomic Radiation) 2000 Report, almost 10 to 20-fold differences in the patient radiation doses happened to occur depending on the medical institutions in the European Union (EC) and OECD countries even though the patients might receive the same diagnostic examination. Contrary to the SFS (Screen Film System) having an approximate gray-scale of 101.5, the flat-panel detector has a grey-scale of 104 and an exposure-index(EI) similar to that of the human eye. Therefore, this flat-panel detector has a wide range of exposure latitudes compared to that of the SFS. As this flat-panel detector has a wide dynamic range of the EI, an under- or an over-exposure of the radiation may occur without any image-quality degradation of the radiograph. In fact, the digital radiograph can be obtained even with a much reduced radiation dose down to 30-50% without any diagnostic performance deterioration. However, the patient dose couldn't have been decreased compared to that of the SFS due to its own dose-creep phenomena. Therefore, in this thesis, three kinds of experiments have been done for optimization of the exposure dose of the digital radiography. The first experiment on the 'Research on image quality and effective dose by exposure-index variation', showed that even with a good selection of the EI, the optimized radiograph could be obtained in the radiation examination within the range of 2.5-fold dose difference, not that of several tens-fold. Moreover, the second experiment on the 'Additional area of the filter according to the dose (DAP: Dose Area Product) and the changes in the X-ray energy spectrum' confirmed that under the same test conditions, about 30% reduction of the dose could be obtained just by including an additional filter to absorb the long-wavelength X-ray energy. In addition, in the third experiment, an effective dose conversion factor(CF) has been derived from the correlation between the DAP values and the effective radiation doses. Based on these experimental results including the derived CF, a systematic radiation exposure management system has been developed, and with which the radiation dose exposed to the patients can be effectively controlled for guaranteeing the minimal radiation exposure in the digital radiography. Finally, from the good experimental results, a feasibility of the implemented system has been confirmed in the practical fields.
