Purpose: To evaluate postoperative changes in the strength and endurance of the shoulder rotator muscles after arthroscopic Bankart repair on patients with anterior shoulder instability using isokinetic test. Materials and methods: Twenty-three patients with anterior shoulder instability were retrospectively evaluated. Isokinetic testing to evaluate shoulder rotator strength and endurance of both the affected and contralateral shoulders was performed immediately prior to and 1 year after arthroscopic Bankart repair. Testing was performed at the angular velocity of 60°/sec and 180°/sec, with both 30° and 90° shoulder abduction. Strength (peak torque normalized to body weight at 60°/sec), endurance (total work at 180°/sec), external rotator to internal rotator ratio and percent deficit values were evaluated for each patient. Results: The average postoperative strength of the external rotator muscle at the 30° position was higher than the average preoperative value (24.7±7.3 Nmkg-1100 vs 29.7±8.1 Nmkg-1100, p=0.01). The endurance of the external and internal rotator muscles were significantly increased at 1 postoperative year in both of the shoulder abduction positions tested (ER in 30°: 222.3±87.8 J vs. 284.2±117.4 J, P=0.02; ER in 90°: 225.53±100.5 J vs. 290.0±93.8 J, P=0.037; IR in 30°: 512.7±176.1 J vs. 684.2±208.4 J, P=0.002; IR in 90°: 541.5±241.3 J vs. 655.4±160.0 J, p=0.048). No significant differences were found in the evaluated isokinetic parameters with regard to the degree of shoulder abduction after surgery. Conclusion: At 1 year following arthroscopic Bankart repair, rotator muscle strength would not be restored to levels comparable to uninjured shoulders. However, surgical intervention can substantially improve rotator muscle endurance.
