Fluoroscopy-guided Percutaneous needle biopsy (PCNB) is a simple and safe procedure that is commonly used to determine proper diagnosis. The purpose of this study is to analyze a variety of factors affecting the diagnostic accuracy of the fluoroscopy-guided PCNB. We retrospectively reviewed the medical records, radiologic features, and the pathologic reports of 258 patients who had undergone the fluoroscopy-guided PCNB for the pulmonary lesions. We assessed the size, depth, location, density, necrosis or cavitation, final diagnosis as characteristic factors, and number of needle passes, the position of patients as procedural factors. Factors affecting the diagnostic accuracy were determined by Pearson's chi-square test and logistic regression analysis. There were 164 malignant and 94 benign lesions. Overall diagnostic accuracy was 90.9% for malignant lesions and 88.3% for benign lesions. The diagnostic accuracy increased as the size of the lesion increased (p=0.024). Among the malignancies, the location of the lesion was also a factor affecting the diagnostic accuracy, lesion located in the left lower lobe showed higher accuracy compared with in the lingular and right middle lobes (p=0.009). The diagnostic accuracy increased as the number of needle passes increased (p=0.041). Especially, there was significant difference in diagnostic accuracy between the group with a single needle pass and those with two needle passes (p=0.024).
