The purpose of this study is to investigate how joint mobilization of thoracic cage using Kaltenborn-Evjenth Orthopedic Manual Therapy and Evjenth-Hamberg stretching can improve the respiratory function, spinal curve, spinal movement, balance ability, and physical fatigue of elderly people that have decreased respiratory function due to restrictive lung disease. The research subject includes a total of 30 elderly people with restrictive lung disease and the research period covers from January 13th to November 10th in 2014. The sample was evenly divided in to three groups: joint mobilization group, joint mobilization & stretching group, and joint mobilization & breathing group. The experiment was conducted for eight weeks, with a thirty-minute respiratory exercise for three times a week.
After applying each of the respiratory exercise therapy, the respiratory function of the aged with restrictive lung disease showed different improvement that joint mobilization & breathing group showed most improvement and followed by joint mobilization & stretching group and joint mobilization group. All of FVC, FEV1, and FEV1/FVC significantly improved in joint mobilization & breathing group and joint mobilization & stretching group(p<.05). The enhancing effects of FEV1/FVC of respiratory function was most prominent in joint mobilization & breathing group(p<.05).
The changes of spinal curve and spinal movement, there were statically significant improvement on thoracic and lumbar curves in joint mobilization & stretching group(p<.01) and improvement of thoracic curves showed significant improvement on lumbar curves(p<.05).
As for the spinal curve change and spinal movement change, improvement of thoracic curves showed significant improvement on flexion and extension of thoracic region and extension of lumbar region. Furthermore, improvement of lumbar curves showed significant improvement on extension of lumbar region(p<.05).
The physical fatigue significantly decreased in all of the respiratory exercise therapy(p<.05), joint mobilization & stretching group showed more significant decrease than joint mobilization group and joint mobilization & breathing group(p<.01) and it showing the most efficiency in physical fatigue(p<.05).
This study confirms that joint mobilization of thoracic cage using Kaltenborn-Evjenth Orthopedic Manual Therapy and Evjenth-Hamberg stretching are effective respiratory exercise method that can be used to simultaneously enhancing respiratory function, spinal curve, spinal movement, and physical fatigue among the elderly person in their sixties that have restrictive lung disease.
