In this study, we focused on the fact that the blood concentration of gadolinium should be within a certain range to obtain optimal arterial enhancement. The purpose of our study was to assess the maximum dilution factors for gadolinium-based contrast agents in cerebral CE-MRA. Prior to the patient test, phantom tests were performed to determine the feasibility of diluting the contrast media. The experimental phantoms contained diluted contrast agent depending on the gadolinium concentration. From November 2014 to February 2015, 316 and 302 patients underwent CE-MRA. The two group underwent gadolinium concentration dilution experiment and gadolinium contrast agent dose experiment, respectively. The results were as follows. First, in the phantom test the signal intensity values showed equilibrium from 1000 to 200mmol. It gradually increased as the concentration decreased. It had a peak value at 20mmol and shraply decreased after that(p<.05). Second, in the patient test, the signal intensity values increased from 1000 to 700mmol having the peak value at 700mmol. It decreased after that. The gadolinium concentration that maximize the signal intensity was 650, 700, 750mmol. Third, as a result of Duncan’s post hoc test, the optimal gadolinium concentration which limits osmolality and viscosity to reduce cytotoxic effects maintaining high signal intensity was 700mmol(p<.05). Fourth, on the basis of gadolinium concentration experiment, the gadolinium dose experiment was performed. The signal intensity had peak value at 0.16mL/kg on the 700mmol concentration. The signal intensity was maximize at 0.15 and 0.16mL/kg.
