To overcome it, polymerase chain reaction(PCR) has emerged as an alternative one, and it is known as very sensitive and specific way. However, in practical using, some cases have negative results of PCR from tissue, even though much information is indicative of TB. This study was planned to assess the sensitivity and the specificity of PCR from tissue by comparing the results of different methods to detect TB. MATERIALS AND METHODS: 590 cases were selected with tissue specimens used for nested PCR targeting IS6110 gene. Those cases had studied TB by more than one methods; AFB smear, mycobacterial culture, and PCR using fluid sample, tuberculosis specific antigen induced interferon gamma, and AFB stain from tissue sample. RESULTS: According to our data, the PCR from fluid has higher sensitivity than the PCR from tissue(72.2% versus 51.1%) Instead, the sensitivity of PCR from tissue became higher on specimens with necrotizing granuloma(65.5%), and specimens with necrosis only(66.7%). CONCLUSION: PCR from tissue is the good alternative method to detect TB, because it shows relatively good concordance with culture result. However, our data shows that PCR from body fluid have higher sensitivity than PCR from tissue. Also, when using tissue specimen for PCR, the possibility and sensitivity of detecting TB is higher on specimens with necrotizing granuloma, and we can tell that those with necrosis only is also good for PCR, so it's necessary to perform PCR on the tissue with necrosis only. As comparing to other methods, AFB stain on tissue specimens shows very low sensitivity, and interferon gamma assay has high sensitivity, but low specificity, which makes it difficult to use the latter as rule out test for TB.
