Recently, studies have been conducted on various balance rehabilitation training to improve the balance ability of the elderly, and diverse balance rehabilitation training equipment is being developed. However, there is little research on an optimized strategy for balance rehabilitation training that can maximize the improvement of balance ability in the elderly. Therefore, as a basis to provide an optimized strategy for balance rehabilitation training, we performed this study to analyze the balance characteristics of the user, according to the major rotation of the base plate, of balance rehabilitation training equipment. On the basis of the results, we present a basic strategy for optimizing the possibility of improving balance ability in the elderly with the use of such equipment. To achieve this, seven young male adults with no musculoskeletal or nervous system-related diseases (age: 25.5 ± 1.7 years, average height: 173.9 ± 6.4 cm, average weight: 71.3 ± 6.5 kg, average body mass index: 23.6 ± 2.4 kg/m2) were selected, and the test was conducted by using a balance rehabilitation training equipment that we have customized. In the test, rotation of the base plate of the balance rehabilitation training equipment was controlled to induce the participant’s dynamic rotation. The participant’s four dynamic rotations induced by rotation of base plate were anterior-posterior (AP), right diagonal (RD), media-lateral (ML), and left diagonal (LD). At the same time, by using an eight infrared camera-based three-dimensional motion capture system (T-10s; VICON Motion System Ltd., UK) and the Pedar Flexible Insoles System (Novel GmbH, Munich, Germany), the participant’s major lower-extremity joint angles, center of body mass (COM), and center of pressure (COP) were measured. At same time, surface wireless EMG sensors measured muscle activation from eight muscles (Rectus Femoris (RF), Vastus Lateralis (VL), Vastus Medialis (VM), Tibialis Anterior (TA), Biceps Femoris (BF), Lateral Gastrocnemius (LG), Medial Gastrocnemius (MG) and Soleus (SOL)). We confirmed that different changes of the joint angles in the lower extremity were induced overall (p < 0.05). According to change of joint angles, muscle activities also represented different changes significantly in each dynamic rotation of participants (p < 0.05). In the case of the maximum deviation percentage (one of the balance characteristics), AP and ML dynamic rotation showed a relatively higher maximum deviation percentage than the other dynamic rotation (p 0.05). These results indicate that optimizing the rotation control of the base plate of the balance rehabilitation training equipment, to induce the participant’s AP and ML dynamic rotation with a higher frequency, may maximally increase the possibility of improving balance ability in the elderly. However, for a more accurate strategy for optimizing the rotation control of the base plate of the balance rehabilitation training equipment, we believe that additional tests by using various angular ranges of the base plate’s rotations and speed, as well as analysis of balance characteristics considering a balance index that reflects the muscle activity and cooperative characteristics, need to be conducted.
