본 연구의 목적은 자폐 범주성 장애 아동에게 Cognitive Orientation to daily Occupational Performance (CO-OP) 중재를 적용하여 작업수행 향상에 효과가 있는 지 살펴보고, 중재 동안 자폐 범주성 장애 아동이 작업수행 문제를 해결하기 위해 서 사용하는 영역-구체화 전략과 기법들을 살펴보고자 한 것이다. 또한 학습된 기술이 다른 상황이나 과제로 일반화 및 전이가 되는지 알아보고자 하였다. 본 연구는 자폐 범주성 장애 진단을 받은 만 5∼12세 아동 13명을 대상으로 하였으며, 제비뽑기를 통하여 실험군과 대조군으로 무작위 할당하였다. 실험군에게 는 개별적으로 CO-OP 중재를 1회 60분씩 총 10회기 제공하였으며, 대조군에게는 CO-OP 중재를 제공하지 않았다. 운동-기반 과제 수행과 운동능력의 변화를 살펴 보기 위해 캐나다작업수행측정(Canadian Occupational Performance Measure; COPM), 수행의 질 평가 척도(Performance Quality Rating Scale; PQRS)와 브로닉스-오세레츠스키 운동 능력 검사(Bruininks-Oseretsky Test of Motor Proficiency-2; BOT-2)를 시행하였다. 연구 결과 CO-OP 중재를 받은 실험군은 대조군에 비해 중재 전·후 훈련 과제뿐만 아니라 비훈련 과제에 대해서도 COPM 수행도 및 만족도, PQRS 점수 변화에서 통계학적으로 유의미한 향상을 나타내었다(p<.05). 대조군에서도 훈련 과제 및 비훈련 과제의 COPM 수행도 및 만족도, PQRS 점수의 향상이 있었으나 이는 통계학적으로 유의미하지는 않았다(p>.05). 운동 능력의 변화를 살펴보면, CO-OP 중재를 받은 실험군에서는 BOT-2의 전체 8개 소검사 중 6개 소검사에서 통계학적으 로 유의미한 향상을 나타내었다(p<.05). 대조군에서도 각 소검사의 점수가 향상되었지만, 그 변화가 통계학적으로 유의미하지는 않았다(p>.05). 또한, CO-OP 중재 동안 자폐 범주성 장애 아동들이 작업수행 문제를 해결하기 위해 영역-구체화 전략 중 과제 구체화/수정 전략을, 기술 습득을 촉진시키기 위해서는 강화 기법이 가장 많이 이용되었다는 것을 알 수 있었다. 본 연구를 통하여 CO-OP 중재가 자폐 범주성 장애 아동의 운동-기반 작업수행을 향상시키는데 효과적인 중재 방법임을 입증하였으며, 비훈련 과제의 COPM 수행도 및 만족도와 PQRS 점수의 유의미한 향상을 통하여 습득된 기술의 일반화 및 전이를 살펴보았다는데 의의가 있겠다.

The aim of this study was to apply Cognitive Orientation to daily Occupational Performance (CO-OP) to children with autism spectrum disorder (ASD) to investigate the effectiveness in the improvement on motor-based goals performance, and to identify domain-specific strategies (DSSs) and techniques used by children with autism spectrum disorder to solve motor-based occupational performance problems during the intervention. In addition, this study attempted to find out whether learned skills became generalized or transferred in other situations or tasks. The subjects of this study were 13 children between ages 5~12, with the diagnosis of ASD. They were randomly assigned to either experimental group or control group. Only the experimental group received CO-OP intervention that was 60 minutes per session for a total of 10 sessions, whereas the CO-OP intervention was not provided to the control group. In order to identify the changes of motor-based occupational performance and motor ability, Canadian Occupational Performance Measure (COPM), Performance Quality Rating Scale (PQRS), and Bruininks-Oseretsky Test of Motor Proficiency-2 (BOT-2) was tested. The results showed statistically significant (p.05). When we examine the change in motor ability, the experimental group, who received CO-OP intervention, showed statistically significant improvement in 6 out of the 8 total subtests in BOT-2 (p.05). Furthermore, reinforcement technique to promote task specification/modification strategy and skill acquisition in DSSs was used the most to solve motor-based occupational performance problems in children with autism spectrum disorder during CO-OP interventions. Through this study, CO-OP intervention is an effective intervention method to improve motor-based goals performance in children with autism spectrum disorder. The results are also meaningful in the aspect of examining the generalization and transfer of learned skills through significant improvement in performance and satisfaction of COPM and PQRS scores of untrained tasks.
