(3) 투명렌즈 착용 7일 후에는 리포칼린이 정상안보다 건성안에서 많았으나 써클렌즈 착용 시 정상안보다 건성안에서 분비량이 적었다. 본 연구를 통해 렌즈 재질의 특성과 착색여부에 따라 정상안과 건성안이 렌즈 착용 시 착용 초기와 착용 적응 후 눈물 단백질 분비량뿐만 아니라 조성의 변화 또한 다르게 나타남을 알 수 있었다. 따라서 렌즈재질의 특성으로 인한 정상안과 건성안에서의 눈물단백질 조성 변화에 대한 후속연구가 뒤따른다면 렌즈착용으로 인한 부작용의 감소에 도움이 될 것으로 판단되었다.

The purpose of this study was to investigate the change in tear film protein induced by conventional hydrogel lens wear in normal and dry eyes. Twelve normal eyes and 6 dry eyes were fitted with contact lenses of different materials (polymacon, low water content, non-ionic; hilafilcon B, high water content, non-ionic; etafilcon A, high water content, ionic; and circle etafilcon A, ionic high water content) for an each week. Tears were collected before, after 1 day, and after 7 days of wearing contact lens by using Schrimer strips. Tear samples were analyzed by one-dimensional sodium dodecyl sulfate-polyacrylamide gel electrophoresis and quantified on each band by using the Quantity One software program. The antibacterial proteins (lactoferrin, lipocalin, and lysozyme) in normal eyes were significantly affected by different materials in clear contact lens, especially high water ionic contact lens, while slgA, albumin, and γ-globulin levels were increased on the first day and then returned to before circle contact lens wear in 7 days. The tear proteins in the dry eyes tended to be influenced by the lens with low water non-ionic content among the clear contact lens. The levels of the antibacterial proteins (lactoferrin, lipocalin, and lysozyme) were significantly decreased when wearing circle contact lens over time. The amounts of tear proteins in the normal eyes were as follows: high water content lens wear > low water content wear, non-ionic > ionic, clear lens < circle lens for both 1 day and 7 days. The patterns of tear protein composition were similar between 1 day and 7 days, while the disparity in the amount of tear proteins was higher in 7 days than in 1 day, depending on the lens materials. In contrast with normal eyes, the amount of antibacterial proteins in dry eyes were higher in “high water content” lens wear than in “low water content” lens wear on the first day. However, no significant differences were observed between “high water content” lens wear and “low water content” lens wear, and an inverse tendency was even observed after 7 days. The amount of tear protein in dry eyes wearing non-ionic contact lens was higher than that in dry eyes wearing ionic contact lens on the first day, whereas its composition patterns did not significantly differ between the two lenses or were even higher in ionic contact lens wear at 7 days. (3) The amount of lipocalin was higher in clear etafilcon A lens wear in dry eyes than in circle etafilcon A lens wear in normal eyes but lower in clear etafilcon A lens in dry eyes than in cosmetic etafilcon A lens wear in normal eyes. These results lead to the conclusion that the changes in tear protein composition and amount of tear proteins in normal and dry eyes could be affected by different contact lens materials during initial and late adaptation. This indicates that reduced antibacterial proteins could make the wearer susceptible to bacterial infection or inflammation during different types of contact lens wear. Furthermore, elucidation of the mechanism of these alterations in tear protein caused by contact lens wear could be helpful to alleviate the bacterial infection and inflammation caused by contact lens wear.
