In this point of view, the changes from the osteoarthritis such as subchondral cysts should begin to appear on the weight bearing portion, that is, the lateral part of the acetabular dome and femoral head underneath. On the other hand, subchondral cysts around the fovea capitis are often encountered especially in early stage of osteoarthritis of dysplastic hip joint, which can’t be explained by the above mechanism. In this study, we tried to reveal the mechanism of subchondral cyst formation adjacent to the fovea capitis of femoral head in the dysplastic hip as well as its effect on the progression of the osteoarthritis.
Materials and Methods: 257 hip joints of 135 patients who had surgical treatment for the dysplastic hip joint were included to be reviewed retrospectively as to the existence of subchondral cysts around the fovea capitis. Simple radiographs and 3 dimensional computed tomography (3D CT) were taken in all cases, and magnetic resonance arthrogram (MRA) was obtained in 123 joints. The subchondral cysts were defined by 3D CT to be discernable radiolucent area on at least two different planes among three planes: coronal, axial and sagittal plane, and spur formation around the fovea capitis was identified on 3D CT. Pathology of the ligamentum teres was classified on MRA as intact, hypertrophy, partial tear and complete tear.
Results: Subchondral cysts around the fovea capitis were detected in 100 out of 257 hip joints, which were composed of 39 cases of Tönnis grade 0 and 150, 41, 27 cases of grade 1, 2, 3 respectively. 152 joints showed the spurs that extended more than half in depth of the fovea capitis, while 82 joints were identified to have partial or complete tears of the ligamentum teres. Comparing 100 joints with the medially located cysts (experimental group) to the others (control group), 89 joints (89%) of the experimental group had mild osteoarthritis such as Tönnis grade 0 or Ⅰ, which 100 joints (63%) had in the control group, showing statistically significant difference (p=0.000). When defining the prominent spur as the one occupies more than half in depth of the fovea capitis, 95 cases out of 100 cases of the experimental group (95%) showed the prominent spurs whereas 57 cases out of the 157 control groups (36%) showed them, which means significant difference between the groups (p=0.000). When it came to injury of the ligamentum teres, all 58 cases of the experimental group (100%) had partial or complete tears while 24 cases out of the 65 control groups (37%) had them, also proving significant difference (p=0.000).
