목적: 체구가 작은 1세 이상의 심정지 소아 환자에게 심폐소생술을 시행할 경우 한 손 가슴압박법을 사용하되 환자의 체구가 크거나 구조자의 체격이 작을 경우 두 손 가슴압박법을 이용하도록 권장된다. 그러나, 두 손 가슴압박법이 표준 심폐소생술로 여러 차례 연구가 이뤄진 것이 비해 한 손 가슴압박법에 대해서는 체계적으로 연구가 시행된 바 없었다. 따라서, 한 손 가슴압박법을 시행할 때 시간의 경과에 따른 가슴압박 질의 저하 (예, 가슴압박 깊이의 감소) 정도를 확인하고, 질 저하를 막을 수 있는 전략을 평가해보고자 하였다. 방법: 첫 번째 연구에서는 바닥에 눕힌 가상 심정지 소아 환자 (마네킨)에게 2분 동안 연속된 한 손 가슴압박을 시행하면서 가슴압박 깊이와 속도를 30초 단위 평균값을 측정하여 시간 경과에 따른 변화가 있는지 확인하였다. 40명의 의사가 참여하였고 가슴압박 깊이와 속도는 가속도측정기를 응용한 되먹임 장비를 이용하여 측정하였다. 두 번째 연구는 병원내 소아 심정지 모델을 이용하여 침대 위에 눕힌 가상 심정지 소아 환자 (마네킨)에게 2분간 연속된 한 손 가슴압박을 시행하되 30초 단위로 가슴압박을 시행하는 손을 반대편 손으로 교대하는 실험 1과 손을 교대하지 않는 실험 2를 30분의 휴식시간을 포함한 무작위 교차설계 시뮬레이션 실험으로 진행하면서 가슴압박 깊이와 속도를 측정하여 일정 시간 간격으로 가슴압박을 시행하는 손을 교대하는 것이 시간 경과에 따른 가슴압박의 질 저하를 줄일 수 있는지 확인해보았다. 두 번째 연구에는 30명의 의료인 (의사 및 간호사)이 참여하였다. 세 번째 연구에서는 가슴압박과 인공호흡을 30:2의 비율로 시행하는 병원 밖 소아 심정지 모델을 이용하여 5분동안 한 손 가슴압박을 시행할 때 시간 경과에 따른 가슴압박 질의 변화 (기준실험)를 먼저 확인하고, 이어서 먼저 한 손 가슴압박법을 이용하여 심폐소생술을 시작한 뒤 구조자가 지쳐서 더 이상 효과적인 가슴압박을 시행하기 어렵다고 주관적으로 판단될 때 두 손 가슴압박법으로 가슴압박의 방법을 전환하는 실험 1 (현재 표준 심폐소생술 방법)과 30:2 1주기 마다 가슴압박을 시행하는 손을 교대하면서 한 손 가슴압박법을 지속하는 실험 2를 무작위 교차설계 시뮬레이션 실험으로 진행하였으며 50명의 의과대학생이 참여하였다. 네 번째 연구는 가슴압박을 시행하는 손을 교대함으로써 발생할 수 있는 가슴압박 중단시간을 최소화하기 위해 기존의 한 손 가슴압박법의 자세를 환자의 가슴압박 지점에 구조자의 체중을 더 효과적으로 싣고, 압박기간 중 안정된 자세를 유지할 수 있도록 한 손 가슴압박법의 자세를 변형한 뒤 그 효과를 확인해보았다. 우선 환자의 가슴을 압박하는 손의 장축을 압박지점 (가슴뼈의 아랫쪽 1/2 지점)에 수직으로 위치시킨 뒤, 반대편 손으로 가슴을 압박하는 손의 팔꿈치 관절을 잡아서 가슴압박을 시행하는 동안 팔꿈치 관절이 굽혀지지 않도록 고정시키는 방식으로 한 손 가슴압박법의 자세를 변형하였으며 이 자세를 변형 한 손 가슴압박법이라고 명명하였다. 실험은 두 번째 연구와 동일한 병원내 소아 심정지 모델을 이용하였고, 2분간 변형 한 손 가슴압박법을 연속하여 시행하면서 가슴압박 깊이와 속도가 시간 경과에 따라 변하는지 확인하였다. 네 번째 연구에는 30명의 의사가 참여하였다. 결과: 첫 번째 연구에서 평균 가슴압박깊이는 시간 경과에 따라 유의하게 감소하였으나 (0–30초, 44.3 ± 4.1 mm; 30–60초, 42.4 ± 4.9 mm; 60–90초, 40.5 ± 5.8 mm; and 90–120초, 38.7 ± 5.7 mm; P < 0.01), 평균 가슴압박속도는 변화가 없었다. 두 번째 연구에서도 평균 가슴압박깊이는 30초 간격으로 가슴을 압박하는 손을 반대편 손으로 교대한 경우 (실험 1)와 교대하지 않은 경우 (실험 2) 모두 시간 경과에 따라 유의하게 감소하였다 (0–30초, 43.4 ± 7.4 대 42.8 ± 7.6 mm; 30–60초, 42.8 ± 8.7 대 40.3 ± 8.8 mm; 60–90초, 40.5 ± 8.9 대 38.2 ± 9.6 mm; 90–120초, 40.2 ± 10.2 대 36.9 ± 9.7 mm; P < 0.01). 그러나, 첫 번째 30초 구간을 제외한 모든 구간에서 손을 교대한 실험 1의 평균 가슴압박깊이가 교대하지 않은 실험 2에 비해 유의하게 깊었다 (P < 0.05). 평균 가슴압박속도는 시간에 따라 변하지 않았고 두 실험 간에 차이도 없었다. 세 번째 연구에서 손교대를 시키지 않은 기준실험의 경우 시간 경과에 따라 평균 가슴압박깊이가 유의하게 감소하였으나 (0–1분, 44.5 ± 5.3 mm; 1–2분, 43.7 ± 6.1 mm; 2–3분, 43.4 ± 6.5 mm; 3–4분, 43.2 ± 6.5 mm; 4–5분, 42.3 ± 6.5 mm; P <0.05), 먼저 한 손 가슴압박법을 사용하다가 두 손 가슴압박법으로 전환한 실험 1과 30:2 1주기마다 가슴압박을 시행하는 손을 반대편 손으로 교대한 실험 2에서는 변하지 않았다. 또한, 마지막 4–5분 구간에서는 기준실험의 평균 가슴압박깊이가 실험 1 (44.5 ± 5.1 mm, P < 0.01)과 실험 2 (44.2 ± 6.1 mm, P < 0.01)에 비해서 유의하게 낮았다. 실험 1과 실험 2의 평균 가슴압박깊이는 전구간에서 차이가 없었다. 네번째 연구에서는 변형 한 손 가슴압박법을 사용했음에도 평균 가슴압박깊이가 시간 경과에 따라 유의하게 감소하였다 (0–30초, 43.3 ± 3.7 mm; 30–60초, 42.0 ± 4.3 mm; 60–90초, 41.0 ± 4.8 mm; 90–120초, 40.4 ± 4.9 mm; P < 0.01). 그러나, 첫 번째 연구와 두 번째 연구의 결과와는 달리 60–90초 구간과 90–120초 구간 사이에서는 평균 가슴압박깊이가 감소하지 않았으며 (P=0.173), 2분간 평균 가슴압박깊이의 감소가 2.9 mm로 제한되었다. 평균 가슴압박속도는 시간 경과에 따라 변하지 않았다. 결론: 연속된 한 손 가슴압박법은 바닥에서 또는 침대 위에서 시행할 때 모두 30초 이후부터 가슴압박깊이가 유의하게 감소하였으나 바닥에서 가슴압박과 인공호흡을 30:2로 시행하는 조건에서는 4분 이후에 유의하게 감소하였다. 가슴압박을 시행하는 손을 교대하는 방법은 연속된 한 손 가슴압박법과 가슴압박과 인공호흡을 30:2로 시행하는 한 손 가슴압박법 모두에서 시간 경과에 따른 가슴압박깊이 감소를 줄일 수 있었으며, 변형 한 손 가슴압박법 또한 가슴압박깊이가 저하되는 것을 지연시킬 수 있었다.

Purpose: Four prospective studies were conducted to evaluate the fatigue effects of the one-handed chest compression technique (OHCC) and to develop strategies for enhancing the quality of OHCC. First, whether compression depth was affected within 2 min when performing OHCC was determined. Second, whether switching hands could delay the decrease in chest compression depth during in-hospital continuous OHCC was determined. Third, the decrease in chest compression depth during out-of-hospital 30:2 compression-to-ventilation ratio OHCC was evaluated, and whether switching hands every other cycle could maintain compression depth was determined. Finally, whether modification of the OHCC posture could maintain the compression depth was determined. Methods: In study 1, 40 physicians performed continuous OHCC on a child manikin lying on a hard floor for 2 min. In study 2, 30 healthcare providers performed continuous OHCC with alternating compression hands every 30 s for 2 min using an in-hospital pediatric arrest model (test 1) and continuous OHCC without hand change (test 2) randomly (crossover design). In study 3, 50 medical students performed 30:2 compression-to-ventilation ratio OHCC for 5 min using an out-of-hospital pediatric arrest model (baseline test), and the compression technique was changed from the OHCC to the two-handed chest compression technique (THCC) when they became subjectively fatigued (test 1) and the compression hand was alternated every other cycle (test 2). In study 4, the posture of the OHCC was modified as follows: first the axis of the rescuer’s compression hand was adjusted to the lower half of the patient’s sternum; then the opposite hand was wrapped around the elbow joint of the rescuer’s compression arm. Thirty physicians performed 2 min of continuous OHCC with posture modification using an in-hospital pediatric arrest model. Compression data, such as mean compression depth (MCD) and mean compression rate (MCR), were recorded using an accelerometer. Results: In study 1, the MCD values decreased significantly over time (0–30 s, 44.3 ± 4.1 mm; 30–60 s, 42.4 ± 4.9 mm; 60–90 s, 40.5 ± 5.8 mm; and 90–120 s, 38.7 ± 5.7 mm; P < 0.01). The MCR values did not change significantly. In study 2, the MCDs at 30 s intervals changed significantly over time in tests 1 and 2 (0–30 s, 43.4 ± 7.4 vs. 42.8 ± 7.6 mm; 30–60 s, 42.8 ± 8.7 vs. 40.3 ± 8.8 mm; 60–90 s, 40.5 ± 8.9 vs. 38.2 ± 9.6 mm; and 90–120 s, 40.2 ± 10.2 vs. 36.9 ± 9.7 mm; P < 0.01). However, with the exception of the first 30 s interval, the MCD values in test 1 were significantly higher than those in test 2 (P < 0.05). The MCRs at 30 s intervals did not significantly change and were not different between the two tests. In study 3, the MCD changed significantly during the baseline test (0–1 min, 44.5 ± 5.3 mm; 1–2 min, 43.7 ± 6.1 mm; 2–3 min, 43.4 ± 6.5 mm; 3–4 min, 43.2 ± 6.5 mm; and 4–5 min, 42.3 ± 6.5 mm; P <0.05). However, no significant differences were observed during test 1 or test 2. The baseline MCD value for the 4–5 min interval was significantly lower than those in test 1 (44.5 ± 5.1 mm, P < 0.01) and test 2 (44.2 ± 6.1 mm, P < 0.01). No differences in the MCDs at any interval were observed between test 1 and test 2. In study 4, the MCD changed significantly over time (0–30 s, 43.3 ± 3.7 mm; 30–60 s, 42.0 ± 4.3 mm; 60–90 s, 41.0 ± 4.8 mm; and 90–120 s, 40.4 ± 4.9 mm; P < 0.01). However, it did not decrease significantly between 60–90 s and 90–120 s (P=0.173). The total decrease in MCD was 2.9 mm over a 2 min period. The MCR did not change significantly over time. Conclusions: Compression depth decreased significantly after 30 s when performing continuous OHCC on the floor in an in-hospital setting. However, it decreased after 4 min during performance of OHCC with a 30:2 compression-to-ventilation ratio on the floor. Use of the alternating-hand technique during continuous and 30:2 compression-to-ventilation ratio OHCC and posture modification during continuous OHCC could delay the decrease in compression depth.

In study 4, the MCD changed significantly over time (0–30 s, 43.3 ± 3.7 mm; 30–60 s, 42.0 ± 4.3 mm; 60–90 s, 41.0 ± 4.8 mm; and 90–120 s, 40.4 ± 4.9 mm; P < 0.01). However, it did not decrease significantly between 60–90 s and 90–120 s (P=0.173). The total decrease in MCD was 2.9 mm over a 2 min period. The MCR did not change significantly over time. Conclusions: Compression depth decreased significantly after 30 s when performing continuous OHCC on the floor in an in-hospital setting. However, it decreased after 4 min during performance of OHCC with a 30:2 compression-to-ventilation ratio on the floor. Use of the alternating-hand technique during continuous and 30:2 compression-to-ventilation ratio OHCC and posture modification during continuous OHCC could delay the decrease in compression depth.
