Understanding of HIV/AIDS trend and epidemics are important epidemiological data for public health, but it is not possible to count the exact number of how many people living with HIV or newly infected or died of AIDS. According to the latest Korean HIV/AIDS annual report in 2014, more than 10 thousand people have been infected since the 1980s, and increasing every recent year rapidly. Reported number of HIV/AIDS infection in Korea is relying on the test result by personal voluntary or medical examination. Due to low testing rate(11% in 2015), it is assumed that there is the huge gap between the reported HIV infection cases and actual infections in the national population. However, there was no research paper to evaluate this gap during last ten years. So the recent trend of HIV infection and epidemics in Korea has been officially unknown. The purpose of this study is to estimate recent HIV infection and epidemics of Korean adults and future trend. To estimate the size of actual HIV infection, MoT(Mode of transmission) which is developed by UNAIDS have used for this study as a statistical estimation model. The epidemiological mode of transmission(MoT) is the mathematical model for HIV estimation based on the common risk behavior among the populations. Due to lack of consolidated HIV risk behavior data in Korea, all recent survey reports have been reviewed to make a strong justification for behavioral attributes of each risk groups. Collected risk data was processed and calculated by MoT spreadsheet which is provided by UNAIDS. The results of the model showed that HIV population among the Korean adult in 2015 was estimated as 19,485(10,913-33,180), and assumed that all infection has transmitted by sexual contact. Major infection risk was sexual contact between the MSM, and 99% of the estimated infection has been occurred in this group. If 14% of condom usage in MSM group were increased 14%, it is estimated that 25.3% of HIV infection will be decreased. HIV population in Korea will be increased continuously by sexual transmission. The strategic approach is necessary how to enhance protected sexual behavior among the major risk groups. On the other hands, it is required that the systemic monitoring data for the risk group and continuous epidemiological research. Epidemiological trend data from high risk behavior groups will contribute to the path forward of the current and future epidemics.
