배경 및 목적 : 복합뇌하수체자극검사는 시상하부-뇌하수체 질환을 가진 환자에서 뇌하수체전엽호르몬의 분비 능을 파악하여 뇌하수체기능의 저하 유무를 판단하는데 중요한 검사 방법으로 확진과 수술 및 방사선 치료 전ㆍ후의 경과 관찰에 도움이 된다.

연구결과 : 표준화된 방법으로 초기 인슐린 용량을 결정하여 투여하고도 목표치 혈당으로 떨어지지 않아 인슐린을 추가 투여한 군은 전체 243명 중 56명(23.1%)이고 인슐린 추가 비투여군은 187명(76.9%)이었다.
인슐린 추가 투여군은 비투여군과 비교하여 남자, 공복혈당, 중성지방, 총 인슐린 투여량이 높았으며, 고밀도지단백콜레스테롤은 낮았다. 단변수분석에서 인슐린 추가 투여군은 남자 38명(37.3%), 여자 18명(12.8%)으로 남자가 많았으며(p<.001), 체질량지수는 인슐린 추가 투여군 남자에서 25kg/m² 이상이 24명(47.1%)으로 비만이 많았다(p=.003). 공복혈당이 100mg/dL 이상 126mg/dL 미만인 공복혈당장애군은 24명(61.5%)이 인슐린 추가 투여를 받았다(p<.001). 중성지방은 인슐린 추가 투여군이 176.1±124.2mg/dL로 비투여군 110.2±60.4mg/dL보다 높았고(p=.001). 총 인슐린 투여량은 인슐린 추가 투여군에서 남자 17.0±4.2U, 여자 13.7±2.6U이고 비투여군는 남자 9.8±2.9U, 여자 7.9±2.8U이었다(p<.001). 그러나 다중로지스틱회귀분석 결과 인슐린 추가 투여 여부와 독립적으로 관련이 있는 요인은 공복혈당 한 가지였다.
성별, 공복혈당, 당뇨병, 체질량지수, 진단명, 중성지방, 고밀도지단백콜레스테롤, HOMA-IR, HOMA-ß를 보정한 모형에서는 공복혈당장애인 경우 공복혈당 100mg/dL 미만에 비해 인슐린 추가 투여군의 오즈비가 6.20배(95% Cl, 1.82-21.14) 높았다.

Backgrounds and Purposes: Combined pituitary function test (CPFT) is an important method to evaluate hypopituitarism by measuring the capacity of anterior pituitary hormone secretion. For the patients with tumorous lesions in a pituitary gland, it is frequently performed not only for the initial diagnosis, but also before and after treatments such as operation and radiation therapy.For a successful and reliable test, adequate hypoglycemia by insulin administration is essential for the evaluation of growth hormone and adrenocorticotropic hormone, however, it is quite common that sufficient hypoglycemia is not induced by standard dose of insulin so that additional administration of insulin is required. We evaluated clinical factors that are possibly associated with insulin resistance.
Research objectives and methods: Data was collected retrospectively by reviewing medical records. Total 364 patients underwent CPFT in a Pituitary Tumor Center from November 2015 until February 2016. Total 243 patients were included to this study who were fully evaluated by our protocol; 102 male patients (42.0%) and 141 female patients (58.0%).
We evaluated various factors that necessitate additional insulin administration in the comparison between characteristics of patients who required additional insulin administration and those of others by t-test, χ²-test and multiple logistic regression analysis. All statistical works were performed by using a SAS 9.3 statistical program.
Research Results: In 56 patients (23.1%),their blood sugar did not reach the target level after initial administration of standard dose of insulin which was decided by a standard protocol whereas sufficient hypoglycemia was induced by single administration of standard dose of insulin in other 187 people (76.9%).
Compared to patients without additional insulin administration, the patients with additional insulin administration showed higher male:female ratio, higher level of fasting blood glucose, higher level of triglycerides and lower level of high-density lipoprotein (HDL) cholesterol. In multivariate analysis, male gender the patients with additional insulin administration included male gender (P<.001) and obesity defined as body mass index greater than 25kg/m² (P=.003). Twenty-four patients (61.5%) were defined as a as their fasting blood glucose ranged from 100mg/dL to 126mg/dL. These patients more frequently required additional insulin administration (p<.001). Patients with additional insulin administration revealed that their triglyceride levels were much higher than in other patients (176.1±124.2mg/dL vs. 110.2±60.4mg/dL, P=.001). ±4.2U and 13.7±2.6U of insulin in the additional insulin administration group whereas men and women needed 9.8±2.9U and 7.9±2.8U of insulin (p<.001) in the non-additional insulin administration group.
