This study is a one-shot case study in which the accuracy of the blood pressures (BPs) measured by 119 emergency medical technician both in a stable indoor situation and in a driving ambulance through palpation and stethoscopes using a blood pressure measurement mannequin BT-CEAB2 (BT Inc., Wonju) was analyzed.
Purpose of the study: The general characteristics, BP measurement related characteristics and knowledge, and self-confidence in BP measurement of 119 emergency medical technician, as well as the accuracy of BPs measured both in a stable situation and in a driving ambulance were compared and evaluated. Study results will be used as base materials for the development of a training program required for improving the accuracy of BPs measured by 119 emergency medical technician.
Study subjects and methods: The general characteristics, BP measurement related characteristics and knowledge, and self-confidence in BP measurement, as well as the accuracy of BPs measured both in a stable situation and in a driving ambulance were compared and evaluated for 31 rescue workers with a first-degree emergency medical technician or nurse certificate during the period of October 11-17, 2016. The collected data were processed for statistics using the SPSS 21.0 program.
Study results: The BP measurement accuracy under general characteristics showed that the group with clinical careers before joining the 119 rescue team had high palpated BP accuracy in a stable situation (p<0.002), the group preferring palpated BP measurement had high palpated BP accuracy in a driving ambulance (p<0.013), and the group with at least one training course attendance had high palpated BP accuracy in a driving ambulance (p<0.022). The BP measurement accuracy under different situations showed that the BPs measured with stethoscope (p<0.002) and palpation (p<0.007) in a stable situation were more accurate than those measured in a driving ambulance with a statistically significant difference. The BP measurement accuracy under self-confidence showed that the group with high self-confidence in palpated BP measurement in a stable situation had high palpated BP measurement accuracy both in a stable situation (p<0.004) and in a driving ambulance (p<0.034). The group with high self-confidence in BP measurement with stethoscopes in a stable situation had high BP measurement accuracy with both stethoscopes (p<0.007) and palpation (p<0.048) in a driving ambulance. The group with high self-confidence in palpated BP measurement in a driving ambulance had high palpated BP measurement accuracy (p<0.000) in a stable situation, showing statistically significant difference.
Conclusion: The BP measurement accuracy of 119 emergency medical technician was higher in a stable situation than in a driving ambulance for BP measurement both with stethoscopes and palpation. The groups with clinical careers and at least one BP-related training course attendance showed the highest accuracy but, in the technical aspect, their accuracy in increasing cuff pressure for BP measurement was low. Further training is required for the BP measurement accuracy improvement and proper BP measurement of 119 emergency medical technician.
