Objective: The aim is to analyse cost-effectiveness of rosuvastatin versus placebo in patients with hypercholesterolemia from the perspective view of health care providers. Methods: I developed an analytical model of the Markov Model for cost–effectiveness analysis comparing rosuvastatin 20mg with placebo. The target population of this study was a both sexes with a mean age of 60 years and korean hypercholesterolemic patients. Before analysing this model, I assumed the two variables such as cost and health outcome. In terms of the cost, Costs of drug, hospital visit, prescription, medical examination, monitoring, treatment were calculated. In terms of health outcome, I assumed health states (healthy, myocardial infarction, stroke, death) and calculated transition probabilities by the year. A lifetime horizon was 5 years and all costs and health outcomes were discounted at 5% annually based on the pharmacoeconomic guideline from health insurance review & assessment service (HIRA). Additionally, it also performed sensitivity analysis. Finally, the results was presented life-year gained and incremental cost-effectiveness ratio (ICER). the estimated ICER used the evidence of decision about willingness-to-pay (WTP) in the pharmaceutical market. According to pharmaceutical surroundings and economic level, that was decided by national threshold. In case of Korea, threshold is 1 GDP (20,000,000 – 24,000,000 KRW). Results: In the base case, the estimated ICER of rosuvastatin was 10,151,056 KRW per life-year gained (LYG) which was a 42.3%-50.8% probablility of being cost-effective. In the sensitivity analysis, the probablilities at WTP thresholds of 1 GDP per LYG were 44.8%-53.7% (3% discount rate) and 39.5%-47.4% (7.5% discount rate) and 37.8-45.4% (–25% treatment effect), 47.9%-57.0% (+25% treatment effect). Conclusions: As a result, clinical use of rosuvastatin was proven cost-effective therapy in patients with hypercholesterolemia whose average age was 60 years.
