재료 및 방법: 제조사 GE Healthcare, 모델명 PETtrace, 양성자 가속에너지 16.5MeV의 Cyclotron 장비를 사용하여 18F 을 제조하였고, 제조사 Siemens Healthcare USA, 모델명 Explora FDG4의 Synthesis module 장비를 사용하여 [18F]FDG를 제조하였다. 실험재료로 [18F]FDG를 방사능농도가 다르게 100mCi/㎖, 200mCi/㎖, 300mCi/㎖, 400mCi/㎖ 준비하였다.
방사능농도, 에탄올농도, 물 희석량, 보관온도, 보관량을 다음과 같이 변화 시켜 방사화학적 순도를 측정하였다.

[18F]FDG의 방사화학적 순도를 증가시키기 위해 [18F]FDG제조 또는 보관 시 방사능농도를 가능한 낮추어야 할 것이고 자유라디칼의 포획제 역할을 하는 에탄올의 첨가가 필요하고 [18F]FDG 보관 시에 가능한 여러 곳에 소분 (분산)해서 보관량을 최소화해야 할 것이다. [18F]FDG제조 시 방사능농도 200mCi/㎖ 이하 경우 방사화학적 순도가 EOS 이후 10시간 동안 KP 및 USP의 기준에 적합하게 유지되었고 방사능농도 200mCi/㎖ 초과 경우 방사화학적 순도를 안정적으로 KP 및 USP의 기준에 적합하게 유지하기 위해 [18F]FDG에 에탄올 첨가가 필요할 것으로 사료된다. 에탄올 농도 0.1%, 0.2%, 0.3% 첨가한 경우 200mCi/㎖ 초과하는 고 방사능농도에서도 방사화학적 순도가 안정적으로 EOS 이후 10시간 동안 KP 및 USP의 기준에 적합하게 유지되는 것으로 나타났지만 잔류용매 에탄올의 발생량을 최소화하기 위해 에탄올농도 0.1% 첨가해야 할 것이다.

The radiochemical purity was measured by varying the radioactivity concentration, ethanol concentration, water dilution, storage temperature and storage amount as described below. The radioactivity concentrations were 100mCi/㎖, 200mCi/㎖, 300mCi/㎖, and 400mCi/㎖, ethanol concentrations were 0.0%(0㎕), 0.1%(1㎕), 0.2%(2㎕), and 0.3%(3㎕), volumes of water dilution(water for injection) were 0㎖, 1㎖, 3㎖ and 7㎖, storage temperatures were 3℃(cooling temperature), 20℃(real storage temperature), 40℃ and storage amounts were 1㎖, 2㎖ and 4㎖. Radiochemical purity was measured using a radio TLC scanner at intervals of 2 hours for 10 hours after completion of [18F]FDG synthesis.
Results and discussions: It might be considered that radioactivity concentration, ethanol addition, storage amount, in this order, had significant effect on the radiochemical purity of [18F]FDG, however, volume of water dilution and storage temperature had no significant effect on the radiochemical purity of [18F]FDG. To increase the radiochemical purity of [18F]FDG, the radioactivity concentration should be as low as possible when preparing or storing [18F]FDG, and the addition of ethanol, which acts as a trapping agent for free radicals, is required and volume of [18F]FDG storage should be minimized by subdividing as much as possible. In the case of [18F]FDG production, the radiochemical purity was maintained to comply with the KP and USP standards for 10 hours after the EOS when the radioactivity concentration was below 200mCi /㎖. When the radioactivity concentration exceeded 200mCi /㎖, ethanol addition to [18F]FDG may be necessary to maintain compliance with USP standards. In case of adding 0.1%, 0.2% and 0.3% of ethanol, the radiochemical purity remained stable even at the high radioactivity concentration exceeding 200mCi /㎖, and it was maintained to conform to KP and USP standards for 10 hours after EOS. In order to minimize the amount of residual solvent ethanol, however, 0.1% ethanol concentration should be added.
Conclusions: When the results of this study are reflected in the manufacture or storage of [18F]FDG, the radiochemical purity can be increased and the radiochemical purity can be maintained stably to comply with the KP and USP standards for 10 hours after EOS, even at high radioactivity concentrations exceeding 200 mCi /㎖. Therefore, the stability of radiochemical purity of [18F]FDG is increased and the reliability of image quality and SUV is improved during PET imaging, so that the accuracy of patient disease diagnosis can be improved.
