This research was conducted to offer theoretical basis for improving the quality of hemodialysis patients’life, especially putting focus on husband and wife, which is dyadic relationship, to figure out hemodialysis patients and spouses’stress, self-efficiency, and social support, and to identify actor and partner effect with dyadic analysis suggested by Kenny. The research was survey research into descriptive causality designed to analyze actor and partner effect: stress, self-efficiency, and social support on the quality of hemodialysis patients and spouses’life. Data of the research were collected from June 1 to August 1, 2016. Subjects of the research were 116 hemodialysis patient-spouse dyads in two offices operated by general practitioners and two general hospitals. The quality of life, self-efficiency, and social support were used as means of the research. The collected data were analyzed with IBM SPSS 21.0 and AMOS 21.0. The results of the research are as follows. 1. In the part of the quality of the subjects’life, hemodialysis patients’ points were 3.13(±0.38), and their spouses’points were 3.22(±0.37), and the quality of the spouses’life was higher than that of patients’ life(t=-2.39, p=.019), which was statistically significant. In the part of stress, hemodialysis patients’points were 2.86(±0.54), and their spouses’points were 2.90(±0.48), which did not show statistical significance. In the part of self-efficiency, hemodialysis patients’points were 3.18(±0.41), and their spouses’points were 3.22(±0.45), which did not show statistical significance. In the part of social support, hemodialysis patients’points were 3.94(±0.69), and their spouses’points were 3.68(±0.84), and the social support perceived by patients was higher than the social support perceived by spouses(t=3.13, p=.002), which was statistically significant. 2. The quality of hemodialysis patients’life had statistically significant correlation with patients’stress(r=-.49, p<.001), their self-efficiency(r=.33, p<.001), their social support(r=.38, p<.001), the quality of their spouses’life(r=.35, p<.001), and spouses’social support(r=.30, p=.001). The quality of spouses’life had statistically significant correlation with spouses’stress(r=-.53, p<.001), their self-efficiency(r=.30, p=.001), their social support(r=.38, p<.001), the quality of patients’life(r=.35, p<.001), patients’ stress(r=-.23, p=.012), and patients’social support(r=.26, p=.004). 3. As a result of analyzing actor and partner effect: stress, self-efficiency, and social support on the quality of hemodialysis patients’life, the quality of patients’ life was directly affected by patients’stress(β=-.25, p=<.001) and their social support (β=.13, p=.003). In the part of size of effect, patients’stress had the highest actor effect on the quality of patients’life(total effect β=-.32, direct effect β=-.25, indirect effect β=-.07), and patients’social support had the second highest actor effect on the quality of patients’life(total effect β=.15, direct effect β=.13, indirect effect β=.02). Spouses’social support mediated by patients’social support had indirect effect on the quality of patients’life(β=.04, p=.034). Patients’stress had direct actor effect on patients’ self-efficiency(β=-.17, p=.016), and patients’social support had direct actor effect on patients’self-efficiency(β=.12, p=.029). Spouses’social support had direct partner effect on patients’social support(β=.24, p=.002). As a result of analyzing actor and partner effect: stress, self-efficiency, and social support on the quality of spouses’life, the quality of spouses’life was directly affected by spouses’stress(β=-.31, p=<.001), their self-efficiency(β=.13, p=039), and their social support(β=.07, p=.049). In the part of size of effect, spouses’stress had the highest actor effect(total effect β=-40, direct effect β=-.31, indirect effect β=-.09), and spouses’social support had the second highest actor effect(total effect β=.10, direct effect β=.07, indirect effect β=.03). Spouses’ stress mediated by spouses’ self-efficiency had effect on the quality of spouses’life(β=-.09, p=.096), and spouses’stress mediated by spouses’social support had indirect effect on the quality of spouses’life(β=.03, p=.068). Patients’stress mediated by spouses’social support had indirect partner effect on the quality of spouses’life(β=-.04, p=.116). Spouses’stress had direct actor effect on spouses’self-efficiency(β=-.24, p=.006). Spouses’stress had actor direct effect on spouses’social support(β=-.53, p=<.001), and patients’stress had direct partner effect on spouses’ social support(β=-.32, p=.022). As above, in the path analysis of effects on the quality of hemodialysis patients’ life and that of their spouses’life, there were differences between patients’and spouses’actor and partner effect. Because actor and partner effect of patients’ stress, spouses’stress, and spouses’social support were high, for improving the quality of life, it can be said that distinctive management program according to patients’and spouses’characteristics needs to be developed and applied. Meanwhile, there was importance of arranging foundational data for providing self-efficiency and social support service and program against negative effects on the quality of life and patients’and spouses’suffering caused by hemodialysis, grave disease. In the research, by identifying actor and partner effect through APIM, relationship between variables which affected the quality of hemodialysis patients’ life and that of their spouses’life could be extended to nursing research methodology and be theoretical basic data for establishing nursing care plan. In other words, there was meaning of being able to compare each size of effect on the quality of hemodialysis patients’life and that of their spouses’life by analyzing actor and partner effect simultaneously through not simple correlation between variables but data of dyadic relationship which had large influence mutually.
Spouses’ stress mediated by spouses’ self-efficiency had effect on the quality of spouses’life(β=-.09, p=.096), and spouses’stress mediated by spouses’social support had indirect effect on the quality of spouses’life(β=.03, p=.068). Patients’stress mediated by spouses’social support had indirect partner effect on the quality of spouses’life(β=-.04, p=.116). Spouses’stress had direct actor effect on spouses’self-efficiency(β=-.24, p=.006). Spouses’stress had actor direct effect on spouses’social support(β=-.53, p=<.001), and patients’stress had direct partner effect on spouses’ social support(β=-.32, p=.022).
