Background: Growth hormone has been used for treatment for the subjects with growth hormone deficiency. It is also used treating short stature of other causes, such as Turner's syndrome, chronic kidney disease and idiopathic short stature. But it is also used for children whose height is lower than 25 percentile to increase their height. The study was intended to compare the growth effect of growth hormone therapy in children according to the degree of height status. Materials and methods: We reviewed retrospectively the medical records of the subjects with idiopathic short stature and the children whose height was lower than 25 percentile from July 2009 to September 2014. These two groups took growth hormone treatment at least for one year. They were all prepubertal children and they had no underlying disease which can cause short stature. We compared height standard deviation score (SDS) and growth velocity before treatment and during 1 year of treatment between two groups respectively. We analyzed factors such as sex, bone age, chronological age, midparental height, height, height SDS, insulin-like growth factor 1 (IGF-I) etc. which might have effect on height SDS and growth velocity. Result: The total number of subjects was 110, of whom 83 subjects were group I (54 boys and 29 girls) and 27 subjects were group II (15 boys and 12 girls). The mean age of group I was 8.5±1.4 years and group II was 8.1±1.4 years. The height SDS in group I after 1 year of treatment was greater than that before treatment (-1.2±0.4 vs -0.5±0.5, P<0.001), and that in group II after 1 year treatment was greater than before treatment (-2.1±0.4 vs. -1.3±0.5, P<0.001). The growth velocity and the height SDS was significantly increased after 1 year treatment in both groups and showed no significant differences. The changes of the height SDS after 1 year treatment showed negative correlation with chronological age before treatment and bone age before treatment Conclusion: These results suggest that growth hormone treatment has a similar effect on increasing height SDS between children with 3rd to 25th percentile and idiopathic short stature. The effect of treatment was better when it was started at younger age.
