Delayed ischemic neurological deficits resulting from cerebral vasospasm is one of the leading causes of morbidity and mortality in patients with SAH. Thus, predicting the outcome of SAH patients at the time of admission is a field of great interest. The second issue of interest is the significant contributors to the development of vasospasm, such as clot density and the rate of clot clearance. In this study, we compared the clot volume and clearance of the two different treatment modalities and its influence on the rate of cerebral vasospasm. The patients admitted to the Asan Medical Center with acute aneurysmal SAH between 2011 to 2014 were retrospectively reviewed. 117 were treated via clip application, and 48 were treated via endovascular coil embolization. A total of one hundred sixty-five patients with aneurysmal SAH were included in this study. The mean age of this patient group was 55 years (range 31-86 years). Sixty-three percent (105/165) of patients were female. Clot clearance rate was higher in patients who underwent craniotomy and clipping. However, the incidence of vasospasm, either symptomatic or radiologic, did not differ between the treatment modalities. Symptomatic vasospasm occurred in 9% of SAH patients, and radiologic vasospasm occurred in 10% of patients. The craniotomy and clipping showed a higher incidence of radiographic vasospasm. Overall, 19% of patients suffered from vasospasm. Even though both groups did not demonstrate the difference in vasospasm rate, the average transcranial Doppler velocity was measured lower in endovascular group. This study indicates that through surgical intervention, higher clot clearance per day can be achieved (1.826mm/day compared to 1.011/day in endovascular coil embolization. However, unlike spontaneous clearance of clots, our results depict the fact that iatrogenic clearance of blood clot does not lead to a lower rate of vasospasm
