There have been numerous studies on the relationship between the acromion types, acromion index and rotator cuff injury. However, most of the studies were performed on a small number of patients with surgical findings. In those studies, the outcome could only show whether there had been a tear or not, and the severity of the rotator cuff injury was not elucidated. We analyzed the relationship between age, gender, the side of the shoulder, acromion type, acromion index and the severity of rotator cuff injury using shoulder magnetic resonance arthrography (MRA). Materials and Methods: A total of 200 shoulder MRA findings were analyzed by radiologist specializing in the musculoskeletal system. The relationship between variables (age, sex, location of the lesion, shape and index of the acromion) and the injury of the supraspinatus (no rupture, partial rupture, full rupture, complete rupture) was confirmed. The partial tear of the supraspinatus tendon was divided into bursal and articular side tear in order to investigate the damage caused by the anatomical difference of the acromion. We also confirmed the differences between single supraspinatus injury and multiple rotator cuff injuries. Results: The severity of the supraspinatus tendon was statistically significant with the older age and right side of shoulder. Multiple rotator cuff injuries were statistically significant with age and acromion type. However, acromion type showed the highest incidence in type 1, and there was no statistical significance in the corrected results. In supraspinatus partial tear, there was no statistical difference between bursal and articular side tears. Discussion: In this study, there was a relationship between age and side of the shoulder, and there was no correlation between acromion type, acromion index and supraspinatus tear. The age at which degeneration could occur also was associated with multiple rotator cuff injuries and is considered to be the most important factor in rotator cuff injury, not anatomical structures such as acromion type or acromion index.
