LPS에 의해 염증이 유도된 인간치주섬유아세포(human gingival fibroblast-1; HGF-1)에서 KPE와 DMF는 MAPKs 신호전달체계 단백질 [phosphorylated extracellular signal-regulated kinases (p-ERK), phosphorylated c-Jun N-terminal kinases (p-JNK), phosphorylated p38 (p-p38)]과 AP-1 형성 단백질인 phosphorylated c-Jun (p-c-Jun)과 c-Fos의 활성을 저해 하였다. 또한, 주요 염증 싸이토카인 interleukin-1β (IL-1β), nuclear factor-kappa B (NF-κB)과 metalloproteinase-2 (MMP-2) 및 MMP-8의 발현을 유의적으로 억제하였다. 더 나아가, KPE와 DMF는 RANKL에 의해 유도된 RAW264.7에서 MAPKs 신호전달체계 단백질 비활성을 통해 파골세포 분화관련 전사인자인 nuclear factor of activated T-cells c1 (NFATc1), c-Fos와 골흡수 효소인 tartrate-resistant acid phosphatase (TRAP), cathepsin K의 발현을 유의적으로 감소시켜 파골세포의 분화를 억제하였다. 세포실험 결과를 기반으로, 치주염 유발 동물 모델을 이용하여 KPE와 DMF의 항치주염 효과를 검증하였다. 12일간 Escherichia coli LPS (10 mg/ml)로 Sprague-Dawley 랫트의 양쪽 하악 어금니잇몸에 치주염을 유발시킨 뒤, 8일간 KPE와 DMF를 매일 경구투여 하였다. 그 결과, KPE와 DMF에 의해 치은조직에서 IL-1β, NF-κB, MMP-2 및 MMP-8과 같은 염증관련 생체지표 발현량이 유의적으로 감소하였다. 치조골에서 KPE와 DMF는 파골세포 분화관련 생체지표인 NFATc1, TRAP 그리고 cathepsin K의 발현량 또한 mRNA 및 단백질 수준에서 모두 효과적으로 감소시켰다. 이를 종합해 볼 때, KPE와 DMF는 LPS로 염증이 유도된 HGF-1에서 염증반응과 RANKL로 유도된 RAW264.7에서 파골세포 분화를 MAPKs 신호전달체계를 비활성화 시킴으로써 억제하였다. 또한 치주염 유발 동물모델에서 KPE와 DMF는 분자생물학적 수준에서 치주염 예방 효과를 보였다. 따라서, KPE와 DMF은 염증반응 억제와 치조골 소실을 효과적으로 예방함으로써 치주염에 대한 천연물 기능성 소재로써 활용될 수 있을 것이라 기대된다.

The periodontitis is an inflammatory disease caused by lipopolysaccharide (LPS), an endotoxin of oral microorganisms. LPS-induced mitogen-activated protein kinases (MAPKs) signaling system activates activator protein-1 (AP-1) complex. The metalloproteinase (MMP) secreted through the activity of the AP-1 complex disrupts the gingiva tissues. In addition, the inflammatory response promotes the secretion of receptor activator of nuclear factor kappa-B ligand (RANKL) in osteoblasts, thereby promoting the differentiation of osteoclasts and causing destruction of alveolar bone. The black ginger (Kaempferia parviflora Wall ex. Baker) used in this study is a representative ginger plant native to Thailand and has been traditionally used to prevent or treat various diseases. K. parviflora possessed anti-cancer, anti-oxidant, anti-obesity; however anti-periodontitis activity of K. parviflora has not been investigated yet. Therefore, this study investigated the inhibitory effects of K. parviflora extract (KPE) and its active compound 5,7-dimethoxyflavone (DMF) on inflammation and osteoclastogenesis. In LPS-induced human gingival fibroblast-1 (HGF-1), KPE and DMF inhibited MAPKs signaling proteins [phosphorylated extracellular signal-regulated kinases (p-ERK), phosphorylated c-Jun N-terminal kinases (p-JNK) and phosphorylated p38 (p-p38)] and AP-1 forming proteins including phosphorylated c-Jun (p-c-Jun) and c-Fos. In addition, KPE and DMF significantly suppressed the expression of major inflammatory cytokines interleukin-1β (IL-1β), nuclear factor-kappa B (NF-κB) and MMP-2. Furthermore, in RAW264.7 differentiated by RANKL, KPE and DMF remarkably inhibited osteoclastic transcription factors such as nuclear factor of activated T-cells c1 (NFATc1), and c-Fos. KPE and DMF also significantly suppressed osteoclastic enzymes including tartrate-resistant acid phosphatase (TRAP) and cathepsin K by regulating MAPKs signaling pathway. Based on the results of in vitro experiments, the anti-periodontitis effects of KPE and DMF were investigated using the animal model with periodontitis. KPE and DMF were orally administrated every day for 8 days after periodontitis was induced by injecting Escherichia coli LPS (10 mg/ml) to bilaterally between the mandible first and second molars periodontium of Sprague-Dawley rats. As a result, the expression of inflammatory biomarkers such as IL-1β, NF-κB, MMP-2 and MMP-8 in gingiva tissues was significantly decreased by KPE and DMF. In alveolar bone, KPE and DMF also markedly decreased in the mRNA and protein levels of osteoclastic biomarkers including NFATc1, TRAP and cathepsin K. Overall, KPE and DMF suppressed osteoclast differentiation in RANKL-induced RAW264.7 and inflammatory response in LPS-stimulated HGF-1 by inactivation of the MAPKs signaling pathway. In the LPS-induced periodontitis on animal, oral treatment of KPE and DMF were also effective of the molecular level. Taken together, it is expected that KPE and DMF can be utilized as natural functional materials for anti-periodontitis by effectively inhibiting inflammatory response and alveolar bone loss.
