Purpose: Recently, the development of radiotherapy equipment and technology has enabled complex and sophisticated treatment, and the accuracy of treatment has become important, and CBCT, which can acquire 3 - D images, is increasingly used. However, in addition to the prescription dose for radiotherapy, there is no management guideline for the exposure dose of this confirmation imaging, and the dose of the radiation dose can not be ignored. In this study, we will present the basic data for dose management and reduction by quantitatively analyzing the exposure dose and presenting the standardized data through the simulation using the Monte Carlo simulation for the exposure dose by CBCT.
Material and Methods: Using the MCNPX code, we simulated the CBCT of the Varian's On-Board Imager (OBI system, V 1.4) and Elekta's X-ray Volumetric Imager (XVI, V R4.5) which are currently operated domestically. First, we tried to secure the reliability by analyzing the spectrum of the simulated CBCT. Then, we calculated the PDD and absorbed dose by simulation using water cube phantom, water cylinder phantom and MIRD phantom.
Result: As a result of the spectrum analysis, the results of the average energy and the characteristic X-ray energy were almost similar when compared with the spectra generated by the CBCT and the SRS-78 program simulated. In PDD measurement experiment using a water cube phantom, the highest value was found at 100% on the surface, and the dose decreased with deeper depth. The measured absorbed dose by water cylinder phantom showed the highest absorbed dose at 0 ~ 1 ㎝ and the dose decreased with deeper depth. The average absorbed dose and organs absorbed dose of head and neck, chest, abdomen, and pelvis were measured using MIRD type phantom. OBI was calculated as 10.90, 25.75, 9.19, 12.23 mGy and XVI was calculated as 10.80, 25.88, 9.25, 12.20 mGy respectively, depending on the site.
Conclusion: In this study, the CBCT spectrum analysis is reliable and used for simulation. In the simulation using water cube phantom and water cylinder phantom, PDD and absorbed dosimetry results are consistent with existing theoretical results and it can be an objective indicator of absorbed dose according to depth. In the simulation using the MIRD type phantom, the organs receiving the maximum and minimum dose according to the site were confirmed, and the average absorbed dose was also confirmed. Based on this simulation, it can be used as basic data for dose management and reduction by presenting standardized data on the exposure dose of tissue by CBCT.
