Background : Rett syndrome, a severe neurodevelopmental disorder mostly affecting female, is characterized by a slowing of development, loss of purposeful use of the hands, distinctive hand movements, delayed brain and head growth, seizures, and intellectual disability. Only few epidemiologic studies of the disease exist worldwide, and the incidence and prevalence of Rett syndrome in South Korea are scarcely reported. This study estimates the prevalence and incidence of Rett syndrome in the Korean population, compares them with those of other similar genetic diseases, and analyze rate of other possible disease-related comorbidities. Methods : This retrospective study included clinical data of all patients diagnosed with Rett syndrome in South Korea. Data were collected from the Health Insurance Review & Assessment Service (HIRA) database from January 2010 to December 2015. For comparison, clinical information of patients primarily diagnosed with Angelman syndrome and Prader-Willi syndrome was also collected. Rett syndrome and other associated factors were defined according to ICD-10 codes. Using the provided data, estimation of incidence and prevalence was calculated on the basis of vital statistics reported by Statistics Korea. Distribution of associated medical conditions was also reported. Results : The average incidence rate of Rett syndrome in South Korea is estimated at 0.45/10,000 (0.84/10,000 for females and 0.08/10,000 for males) within the population for age < 5 years. The prevalence of Rett syndrome (0.80/100,000) in the Korean population was higher than that of Angelman syndrome (0.33/100,000) and Prader-Willi syndrome (0.66/100,000). In terms of associated diseases, 67% of the patients were additionally diagnosed with epilepsy. Gastroesophageal reflux disease and sleep disorders were also observed in 15% and 7% of patients, respectively. Conclusions : These findings will help physicians make appropriate clincal decisions and provide education and counseling that will optimize the patients' health and quality of life. Further research on the epidemiology of Rett syndrome should be conducted. Moreover, a comprehensive approach for various conditions of Rett syndrome is recommended.
