Inappropriate management of medical waste and infection control may increase a risk of medical workers and patients to be exposed to infection and induce biological and chemical environment pollution. This study intended to study actual state and its change of medical waste management and infection control of high risk instrument subject to dental offices for 5 years and identify possible related factors and effects of dental healthcare accreditation on medical waste management and infection control. From Jun. 2017 to Nov. 2017, researchers visited total 82 dental offices including 5 in general hospitals, 8 in dental hospitals, and 69 dental clinics and conducted comparative investigation with their management state 5 years ago through interview in person. For management of solid wastes filtered from spittoon containers and suction pump, removed impression materials with blood stain, and sensor plastic cover with blood stain used for digital radiography, whereas 10 (12.2%), 17 (20.7%), 22 (28.6%) dental offices showed improvement, 8 (9.8%), 4 (4.9%), and 11 (14.2%) dental offices showed deterioration and 57 (69.5%), 51 (62.2%), and 29 (37.7%) dental offices showed same inappropriate management as 5 years ago. In addition, for management of high risk instruments including tooth reduction bur, scaler tip, and water tube, while 11 (13.4%), 25 (30.5%), and 6 (7.3%) dental offices showed improvement, 9 (11.0%), 5 (6.1%), and 6 (7.3%) dental offices showed deterioration and 26 (31.7%), 48 (58.5%), and 63 (76.9%) dental offices showed same inappropriate management still. Meanwhile, for management of used drapes and endo files, it was shown that 5 (6.1%) and 13 (15.9%) dental offices were improved and 68 (82.9%) and 59 (71.9%) dental offices managed appropriately same as 5 years ago. It was found that in accredited institutions with dental healthcare accreditation, all institutions managed medical wastes and high risk instruments appropriately and number of dentists, change of patient number, education experience of medical waste and infection management were factors to improve their management partially. In addition to active systems including preparation of detailed and professional guideline for medical waste management and infection control at dentists, evaluation, management and regular education, the dental healthcare accreditation system should be supplemented and expanded for settlement as effective system for improvement of patient safety and medical service quality.
