Objectives To identify the factors affecting the prognosis of patients with peripheral facial palsy and suggest a classification model for predicting the prognosis with the estimated recovery rate over time. Methods Patients who visited Kyung Hee University Hospital at Gangdong from June 1, 2006 to December 31, 2014 and received East-West Collaboration Treatment (EWCT) with Bell’s palsy and Ramsay-Hunt syndrome were selected according to the eligibility criteria. Data about the change of House-Brackmann Grade (HB Grade) for 2 years from the onset and the factors presumed to be related to the prognosis were derived by retrospectively reviewing the participants’ electrical medical records. First, the estimated recovery rates to HB Grades II and I were calculated using the Kaplan-Meier method. The factors affecting the prognosis of recovery to HB Grade I were determined using univariate Cox regression analysis with a 0.2 cut-off value for age, sex, underlying disease (hypertension, diabetes mellitus), types of diagnosis, palsy side, accompanying symptoms (periauricular pain, auditory impairment, and taste deficit), and axonal loss value of nerve conduction study (NCS). Subsequently, multivariate Cox regression analysis was performed on the selected factors. The factors derived from the Cox regression model were applied to the survival tree analysis model to establish the criteria for the classification of patients with peripheral facial palsy according to prognosis. Finally, the participants were divided based on the derived classification criteria, and the estimated recovery rates for 2 years were determined. Results 768 participants were included in the study after screening 997 patients who met the inclusion criteria. Based on Kaplan-Meier analysis, the estimated recovery rates to HB Grade II and I for the 2 years were 98.2% and 83.3%, respectively. These results showed significantly higher or similar recovery rates in significantly more severe patients compared to previous randomized-controlled studies and observational studies. The univariate Cox regression analysis indicated that 10 factors, including sex, diabetes, hemoglobin A1c, diagnosis, periauricular pain, auditory impairment, taste deficit, initial HB Grade, and average and maximum axonal loss values of NCS, affected prognosis.
