This study aimed to explore the illness experience of adolescents with type 1 diabetes. Using semi-structured questions, twelve T1D adolescents in Korea were interviewed on their illness experiences. Data were analysed using a grounded theory methodology.
Participants described their experience in the core category, ‘being me again’ and it resulted in 13 major categories, 36 sub-categories, and 130 concepts. The categories constructed paradigm model including conditions (causal, context, and intervening condition), phenomenon, and consequences. The causal conditions entail three categories; ‘tied to diabetes management’, ‘feeling restricted’, and ‘being the object of parental supervision.’ The intervening condition was ‘utilizing support system.’ The context condition was ‘surrounded by stigma’ and ‘growing up.’ The phenomenon was ‘sense of wounded self.’ The consequences were ‘the acceptance of diabetes.’
The present study identified the process of the T1D adolescent’s illness experience with four phases; 1) being the otherness 2) closure for defence 3) openness for coping 4) recovering the subjectivity. The process elucidates the importance of the subjectivity for T1D adolescents to establish the diabetes self-management. Given that the initiative-driven management type shows the most desirable adherence, the preservation of subjectivity of adolescents led to the initiative in diabetes management. Other types also were extracted by the type analysis, including temporary acceptance type, parent-dependent management type, and deferred acceptance type.
Findings indicate that illness experience of T1D adolescents should be understood in both physical and psychosocial dimensions, considering the sociocultural and developmental context. The results of this study contribute to provides information for diabetes education. Moreover, the findings encourage nurses to develop intervention programs and strategies to help T1D adolescents.
