Introduction The purpose of this study is to investigate factors that are associated with alveolar bone levels of the distal root of the mandibular second molar, when they are protracted into mandibular first molar space in patients with missing first molar or second premolar with retained deciduous molar. Methods This retrospective study included orthodontic patients whose missing molar or premolar space was orthodontically closed by second molar protraction, with the use of TADs at the SmileWith Dental Clinic. The patients were grouped into the successful group (Group 1, n=43) whose alveolar bone level distal to the second molar was normal, less than 3mm measured from the CEJ, and the compromised group (Group 2, n=8) who showed angular bone defect distal to second molar, with bone level more than 3mm apical to the CEJ. For each patient, panoramic radiographs were collected at pretreatment (T1) and after the full eruption and alignment of the third molar (T2). Measurements were obtained regarding mesial movement and alveolar bone level change of the mandibular second molar, angle change of the second molar, Nolla stage, mesiodistal width, the time of eruption, the vertical change, the horizontal change, and the angle change of the third molar. Results Age, third molar angulation at T1, second molar angulation at T1, and mesial alveolar bone of second molar at T1 were included as final variables, of which age and third molar angulation at T1 showed statistically significant correlations with distal alveolar bone loss of the mandibular second molar. Conclusions When second molars are protracted to close the missing space of mandibular first molar or second premolar, and third molars aligned into occlusion, significant alveolar bone defect may present as a result of treatment. Old age, horizontal angulation of the third molars, and low alveolar bone level in the mesial of second molar may increase the risk of alveolar bone defect in the distal of the second molar after protraction.
