대상 및 방법 본 연구는 제7기 1차년도(2016) 국민건강영양조사를 이용하였으며 조사대상자 10,806명 중 건강설문조사, 검진조사, 영양조사 중 1개 이상 조사에 참여한 20대, 30대, 40대 성인은 2,937명 이었다. 이 중 검진조사와 영양조사 자료가 누락되거나 충분하지 않은 경우(n=355)만 제외하고 20~49세 성인 중 BMI<18.5kg/㎡ 성인 117명은 저체중군, 18.5kg/㎡≤BMI<23kg/㎡ 성인 1,089명은 정상군, 23kg/㎡≤BMI<25kg/㎡ 성인 506명은 과체중군, BMI≥25kg/㎡ 성인 870명은 비만군으로 정의하여 본 연구는 총 2,582명을 대상자로 선정하여 분석하였다. 또한 일반적 특성, 주관적 체형 인식, 1년간의 체중조절 여부, 흡연, 음주, 스트레스인지와 수면시간, 신체활동(1주일간 걷기, 1주일간 근력운동)일수, 식사 및 외식 빈도와 식품섭취빈도조사를 토대로 얻어진 영양소 섭취에 대하여 체질량 지수에 따른 성별의 차이를 비교해 보았으며, 이로 인해 얻어진 지표들과 혈액지질성분과의 관련성 알아보았다.
연구 결과 및 결론 본 연구의 일반적 특성에서 남성이 여성보다 교육수준이 높았으며, 직업은 남성은 관리직이, 여성은 무직이 가장 높은 분포를 보였다. 체질량 지수에 따른 분포에서는 남성의 경우 ‘비만군’이 45.8%로 가장 높게 나타났으며, 이는 여성 24.1%의 약 2배 정도 높은 비율을 보였다. 체질량 지수가 증가할수록 체중과 허리둘레가 증가하였으며, 특히 남, 녀 모두 ‘비만군’에서 허리둘레의 평균이 복부비만의 기준보다 큰 수치를 보여 복부비만도 함께 나타내고 있음을 알 수 있었다. 체질량 지수에 따른 성별 주관적 체형인식에서는 체질량 지수에 따라 남성은 대체적으로 현재 자신의 체형이 기준보다 정상이거나 마르다고 생각했으며, 여성은 반대로 기준보다 과체중이거나 비만이라고 생각한다는 것을 알 수 있었고, ‘1년간 체중 조절 여부’에서는 남성이 여성보다 체중 조절의 노력을 하는 비율이 낮다는 것을 알 수 있었다. 음주빈도에서는 체질량 지수에 따른 모든 집단에서 ‘월1-4회’의 항목에서 가장 높은 분포를 나타내었으며, 한 번에 마시는 음주량에서는 체질량 지수가 높아짐에 따라 음주량이 많아지는 것으로 나타났다. 흡연에서는 체질량 지수에 따른 유의한 차이는 나타나지 않았고, 신체활동에서도 1주일 동안 걷기 횟수의 차이는 유의하지 않았으며, 1주일 동안 근력 운동 횟수의 차이에서도 체질량 지수에 따른 차이는 없었으나 ‘전혀 하지 않는다’는 항목을 제외한 ‘주1-3일’, ‘주4일 이상’의 항목에서 남성이 여성보다 높은 분포를 보여 유의한 차이를 나타냈다. 정신건강에서 주중 하루 평균 수면 시간에서는 남, 녀 간의 유의한 차이는 없었다. 평소 스트레스 인지 정도와의 차이에서는 체질량 지수 별 모든 집단에서 ‘조금 느낀다’라는 항목이 남, 녀 모두 가장 높은 분포를 나타내었으나, 비만군을 살펴보았을 때 남성의 경우 여성보다 스트레스를 인지하는 비율이 조금 낮은 것을 알 수 있었다. 식행동에서 남성이 여성에 비해 외식의 빈도와 저녁 식사의 빈도가 높은 것으로 나타났으며, 유의한 차이를 보였다. 체질량 지수에 따른 영양소 섭취에서는 비만 할수록 대부분의 영양소의 섭취량이 많다는 것을 알 수 있었고, 남성의 연령별 영양소 섭취의 차이에서는 단백질과 지방, 불포화지방산과 콜레스테롤, 나트륨의 경우 한국인 영양권장량보다 높은 섭취율을 보였으며, 이것이 비만을 야기 시키는 식습관과 관련이 있을 것으로 추측할 수 있었다. 여성의 경우 연령이 증가 할수록 지방의 섭취는 줄고 비타민 섭취가 증가하는 것을 알 수 있어, 건강한 식습관을 위해 노력한다는 것을 추측할 수 있었다. 혈압 및 혈액지질성상과 비만지표와의 상관관계에서 허리둘레와 BMI는 혈압 및 혈액지질성상의 수치 상승에 영향을 미치는 것을 알 수 있었다. 건강행태와의 상관관계에서 주관적 체형인식이 비만하다고 느끼고, 체중조절이 노력을 하지 않을수록, 음주량과 흡연량이 많을수록 혈액지질수치가 상승하여 고지혈증과 심혈관질환에 영향을 줄 것이라는 것을 추측할 수 있었고, 신체활동과 수면, 스트레스는 혈액지질성상과 관련성이 없는 것으로 나타났다. 식행동과의 상관관계에서 아침 식사 빈도가 적고, 저녁 식사와 외식의 빈도가 높을수록 혈액지질수치가 상승하는 것을 알 수 있었다. 영양소 섭취와의 상관관계에서 열량, 탄수화물, 단백질 섭취가 증가할수록 혈압, 중성지방 수치가 증가하고, HDL-C는 감소하는 것으로 나타났고, 지방 섭취가 증가할수록 혈압이 증가하는 것으로 나타났다. 포화지방산 섭취가 증가할수록 수축기 혈압은 증가하고 HDL-C는 감소하는 것으로 나타났으며, 나트륨 섭취가 증가할수록 혈압과 TG는 증가하고, HDL-C은 감소하는 것으로 나타났다. TC와 LDL-C, FBS의 경우 상관관계 요인을 나타내는 항목이 없음을 알 수 있었으며, Cholesterol 섭취의 경우 혈압 및 혈액지질성상과 관련성이 없는 것으로 나타났다. 이상의 결과를 종합해 보면 청장년 남성의 비만율이 여성에 비해 2배정도 높다는 점과 비만에 영향을 미치는 요인들(주관적 체형인식과 체중조절 노력 여부, 음주, 저녁 식사 빈도, 외식 빈도 및 영양소 섭취 상태)이 남성에서 더 높게 나타났으며, 이러한 지표들이 혈액지질성상과의 관련성이 있는 것으로 나타났다. 본 연구 결과를 토대로 중장년기 만성질환의 발병과 사망률을 감소시키기 위해 청장년층 남성 비만의 예방과 영양교육이 시급할 것으로 사료된다. 또한 본 연구가 청장년층의 비만 예방과 영양교육의 필요성을 인식시키는 기틀을 마련하는 기초 자료라는 점에 의의가 있다고 생각한다.

Background According to the World Health Organization (WHO), more than 1.9 billion adults, which is about 39% of adults in the world, were overweight in 2016, and 650 million among them were obese. This is known as one of the major risk factors of noncontagious diseases. For the past 10 years, the obesity prevalence in Korean people aged over 30 has been on the rise: 34.8% in 2005, 33.9% in 2010, and 37.0% in 2016. Such increase of the obesity rate has caused prevalence of noncontagious disorders and increase in mortality in Korea. In particular, several studies have revealed that overweight and obesity are two of the four key metabolic changes that aggravate risks of noncontagious diseases such as hypertension, hyperglycemia, and hyperlipidemia, and that increase of blood lipid components has a close correlation with cardiovascular disorders. Thus, it is required to actively manage obesity, which is related to preceding diseases and risk factors, so as to prevent noncontagious diseases in adults. The prevention and management of obesity can be considered to start from young population. In this regard, this research aims to analyze the health behavior and nutrient intakes of young and middle-aged adults in between 20s and 40s who may neglect health care according to body mass index, provide basic materials required for obesity education, and study elements affecting obesity and blood lipid parameters in order to provide basic materials to help recognize the importance of nutrition education for prevention of chronic diseases such as hyperlipidemia and cardiovascular disorders.
Objects and Methods This research utilized the 7th Korea National Health and Nutrition Examination Survey of the 1st year (2016). Among 10,806 research subjects, 2,937 adults in their 20s, 30s and 40s participated in more than 1 studies including health survey, physical examination, and nutrition survey. Excepting for the cases of omitted or insufficient physical examination and nutrition survey data (n=355), a total of 2,582 research subjects were selected and analyzed in the following groups: a low-weight group including 117 adults aged 20-49 with BMI<18.5kg/㎡, a normal group including 1,089 adults with 18.5kg/㎡≤BMI<23kg/㎡, an overweight group including 506 adults with 23kg/㎡≤BMI<25kg/㎡, and an obese group including 870 adults with BMI≥25kg/㎡. For information on nutrient intakes obtained by surveying on general characteristics, subjective perceptions of body shape, efforts to lose weight for a year, smoking, drinking, stress recognition, sleeping hours, days of physical activities (walking for 1 hour, muscle building exercise for 1 hour), meal and eating out frequency, and eating frequency, gender differences according to body mass index were compared, and the result indices were analyzed for the correlation with blood lipid components.
Results and Conclusions The general characteristics of the research subjects revealed that males had higher education levels than females. While management was the most common occupation for men, the majority of the women were unemployed. As for body mass index, ‘the obese group’ showed the highest at 45.8% among the men, which was about double the women’s (24.1%). With higher body mass index, weight and waist circumference increased. In both males and females, in particular, ‘the obese group’ showed higher averages of waist circumference than the standard of abdominal obesity, indicating abdominal obesity as well. When it comes to subjective perceptions of body shape according to body mass index, most of the male subjects considered that they were normal or slimmer than the standard, while female subjects though they were overweight or obese compared to the standard. In the survey on ‘efforts to lose weight for 1 year’ showed that less men had tried to control weight than women. For drinking frequency according to body mass index, ‘1-4 times a month’ was the most answered in all groups; the higher the body mass index, the more the amount of drinks at once. Meanwhile, smoking did not show significant differences according to body mass index. Physical activities also showed no significant differences according to body mass index in both the frequency of walking a week and the number of muscle building exercise a week. However, ‘1-3 days a week’ and ‘more than 4 days a week,’ except for ‘never,’ were more answered by men than women, showing significant gender differences. In psychological health, there was no significant difference between males and females for average daily sleeping hours. As for the level of stress recognition according to body mass index, all groups of the both genders answered the most for ‘I feel stressed a bit.’ When it comes to the obese group, however, fewer men recognized their stress than the counterparts. In eating behavior, a significantly higher proportion of males ate out and had dinner more frequently than females. For nutrient intakes according to body mass index, the more obese, the more intakes of most nutrients. The difference in nutrient intakes of men by age showed that they had more protein, fats, unsaturated fatty acids, cholesterol, and sodium than the Recommended Dietary Allowances for Koreans, and this is assumed to be related to the dietary habits causing obesity. On the other hand, females had less fat and more vitamins as they got old, indicating that they work harder to develop healthy dietary habits. In the correlation between blood pressure, blood lipid parameters, and obesity index, waist circumference and BMI affected the increase of blood pressure and blood lipid parameters. In the correlation with health behavior, those who answered their subjective perceptions of body shape as obese, who made less effort in weight control, and who drank and smoked more showed higher blood lipid levels, indicating that there would be more likelihood of hyperlipidemia and cardiovascular disorders. Meanwhile, it was revealed that there was no correlation between physical activities, sleeping, and stress and blood lipid parameters. In the correlation with eating behavior, the fewer breakfast frequency and the more dinner and eating out frequency, the higher the blood lipid levels. In the correlation with nutrient intakes, the more intakes of calories, carbohydrates, and protein, the higher the blood pressure and neutral fat levels and the lower the HDL-C. The more intakes of fat, the higher the blood pressure. The more intakes of saturated fatty acids, the higher the systolic blood pressure and the lower the HDL-C. The more intakes of sodium, the higher the blood pressure and TG and the lower the HDL-C. As for TC, LDL-C, and FBS, there was no items indicating correlation factors, and intakes of cholesterol had nothing to do with blood pressure and blood lipid parameters. As a result, the obesity rate of young and middle-aged males was about double the rate of females. The male subjects showed higher levels in factors affecting obesity: subjective perceptions of body shape, efforts to lose weight, drinking, dinner frequency, eating out frequency, and nutrient intakes, and such indices were revealed to be related to blood lipid parameters. Based on the results of this research, it is considered urgent to educate young and middle-aged males on obesity prevention and nutrition to reduce chronic diseases and mortality in middle and older-aged people. Furthermore, this research has significance in providing basic materials that develop the cognitive framework for the importance of obesity prevention and nutrition education for young and middle-aged people.

In particular, several studies have revealed that overweight and obesity are two of the four key metabolic changes that aggravate risks of noncontagious diseases such as hypertension, hyperglycemia, and hyperlipidemia, and that increase of blood lipid components has a close correlation with cardiovascular disorders. Thus, it is required to actively manage obesity, which is related to preceding diseases and risk factors, so as to prevent noncontagious diseases in adults. The prevention and management of obesity can be considered to start from young population. In this regard, this research aims to analyze the health behavior and nutrient intakes of young and middle-aged adults in between 20s and 40s who may neglect health care according to body mass index, provide basic materials required for obesity education, and study elements affecting obesity and blood lipid parameters in order to provide basic materials to help recognize the importance of nutrition education for prevention of chronic diseases such as hyperlipidemia and cardiovascular disorders.
Objects and Methods This research utilized the 7th Korea National Health and Nutrition Examination Survey of the 1st year (2016). Among 10,806 research subjects, 2,937 adults in their 20s, 30s and 40s participated in more than 1 studies including health survey, physical examination, and nutrition survey. Excepting for the cases of omitted or insufficient physical examination and nutrition survey data (n=355), a total of 2,582 research subjects were selected and analyzed in the following groups: a low-weight group including 117 adults aged 20-49 with BMI<18.5kg/㎡, a normal group including 1,089 adults with 18.5kg/㎡≤BMI<23kg/㎡, an overweight group including 506 adults with 23kg/㎡≤BMI<25kg/㎡, and an obese group including 870 adults with BMI≥25kg/㎡. For information on nutrient intakes obtained by surveying on general characteristics, subjective perceptions of body shape, efforts to lose weight for a year, smoking, drinking, stress recognition, sleeping hours, days of physical activities (walking for 1 hour, muscle building exercise for 1 hour), meal and eating out frequency, and eating frequency, gender differences according to body mass index were compared, and the result indices were analyzed for the correlation with blood lipid components.
Results and Conclusions The general characteristics of the research subjects revealed that males had higher education levels than females. While management was the most common occupation for men, the majority of the women were unemployed. As for body mass index, ‘the obese group’ showed the highest at 45.8% among the men, which was about double the women’s (24.1%). With higher body mass index, weight and waist circumference increased. In both males and females, in particular, ‘the obese group’ showed higher averages of waist circumference than the standard of abdominal obesity, indicating abdominal obesity as well. When it comes to subjective perceptions of body shape according to body mass index, most of the male subjects considered that they were normal or slimmer than the standard, while female subjects though they were overweight or obese compared to the standard. In the survey on ‘efforts to lose weight for 1 year’ showed that less men had tried to control weight than women. For drinking frequency according to body mass index, ‘1-4 times a month’ was the most answered in all groups; the higher the body mass index, the more the amount of drinks at once. Meanwhile, smoking did not show significant differences according to body mass index. Physical activities also showed no significant differences according to body mass index in both the frequency of walking a week and the number of muscle building exercise a week. However, ‘1-3 days a week’ and ‘more than 4 days a week,’ except for ‘never,’ were more answered by men than women, showing significant gender differences. In psychological health, there was no significant difference between males and females for average daily sleeping hours. As for the level of stress recognition according to body mass index, all groups of the both genders answered the most for ‘I feel stressed a bit.’ When it comes to the obese group, however, fewer men recognized their stress than the counterparts. In eating behavior, a significantly higher proportion of males ate out and had dinner more frequently than females. For nutrient intakes according to body mass index, the more obese, the more intakes of most nutrients. The difference in nutrient intakes of men by age showed that they had more protein, fats, unsaturated fatty acids, cholesterol, and sodium than the Recommended Dietary Allowances for Koreans, and this is assumed to be related to the dietary habits causing obesity. On the other hand, females had less fat and more vitamins as they got old, indicating that they work harder to develop healthy dietary habits. In the correlation between blood pressure, blood lipid parameters, and obesity index, waist circumference and BMI affected the increase of blood pressure and blood lipid parameters. In the correlation with health behavior, those who answered their subjective perceptions of body shape as obese, who made less effort in weight control, and who drank and smoked more showed higher blood lipid levels, indicating that there would be more likelihood of hyperlipidemia and cardiovascular disorders. Meanwhile, it was revealed that there was no correlation between physical activities, sleeping, and stress and blood lipid parameters. In the correlation with eating behavior, the fewer breakfast frequency and the more dinner and eating out frequency, the higher the blood lipid levels. In the correlation with nutrient intakes, the more intakes of calories, carbohydrates, and protein, the higher the blood pressure and neutral fat levels and the lower the HDL-C. The more intakes of fat, the higher the blood pressure. The more intakes of saturated fatty acids, the higher the systolic blood pressure and the lower the HDL-C. The more intakes of sodium, the higher the blood pressure and TG and the lower the HDL-C. As for TC, LDL-C, and FBS, there was no items indicating correlation factors, and intakes of cholesterol had nothing to do with blood pressure and blood lipid parameters. As a result, the obesity rate of young and middle-aged males was about double the rate of females. The male subjects showed higher levels in factors affecting obesity: subjective perceptions of body shape, efforts to lose weight, drinking, dinner frequency, eating out frequency, and nutrient intakes, and such indices were revealed to be related to blood lipid parameters. Based on the results of this research, it is considered urgent to educate young and middle-aged males on obesity prevention and nutrition to reduce chronic diseases and mortality in middle and older-aged people. Furthermore, this research has significance in providing basic materials that develop the cognitive framework for the importance of obesity prevention and nutrition education for young and middle-aged people.
