골격성 Ⅲ급 부정교합은 한국에서 비교적 높은 비율을 차지하고 있으며 특히 성장이 완료된 성인에서는 악교정 수술 또는 절충치료가 이루어지고 있다. 수술을 할 정도로 부정교합의 정도가 심하지 않으며 절충치료 시 발치로 인한 안모의 악화가 예상될 경우에는 미니스크류를 사용해 하악 소구치 발치 없이 하악 전치열을 후방이동시킬 수 있다. 본 연구의 목적은 측모두부방사선사진을 이용하여 골격성 Ⅲ급 성인 환자에서 미니스크류를 이용한 하악 전치열의 후방이동 시 나타나는 치아 이동 양상과 치료 결과의 안정성에 대해 알아보고자 하였다.골격성 Ⅲ급으로 진단받고 하악 구치부 협측에 식립된 미니스크류를 이용하여 하악 전치열의 후방이동을 완료한 성인 환자 17명의 의무기록 및 교정치료 전(T0), 치료 후(T1), 유지(T2) 시점의 측모두부방사선사진을 이용하여 하악 전치 및 구치의 수직 및 수평적 위치와 각도 변화를 분석하고 유지기간 동안의 안정성과 상관관계에 있는 요소를 골격적 요소, 치료 기간 동안의 치성 변화량, 하악 전치부 치조골로 나누어 분석한 결과 다음과 같은 결론을 얻었다. 1. 수직기준선으로부터 수평이동은 치료기간 동안(T1-T0) 하악 중절치(p < 0.05)와 제1, 2대구치(p < 0.001)의 치관이 후방이동 되었다. 유지기간 동안(T2-T1) 하악 중절치는 유의한 변화 없었고, 제1,2대구치는 전방이동(p < 0.05) 되었으며 재발량은 각각 26.8 %, 23.9 % 였다. 2. 수직적으로는 제2대구치가 치료기간 동안(T1-T0) 압하되었다(p < 0.001). 유지기간 동안(T2-T1) 하악 중절치와 제1,2대구치의 유의한 변화는 없었다. 3. 경사이동은 치료기간 동안(T1-T0) 하악 중절치(p < 0.05)와 제1, 2대구치(p < 0.001)는 후방경사 되었다가 유지기간 동안(T2-T1) 하악 중절치와 제2대구치는 유의한 변화 없었고, 제1대구치는 유의하게 전방경사(p < 0.05) 되었으며 재발량은 24.8% 였다. 4. 하악 제1대구치에서 치료 기간 동안 후방이동이 많을수록 유지기간 동안 전방이동량이 컸고(p < 0.01), 치료 기간 동안 압하량이 클수록 유지기간 동안 전방 정출 경향이 컸다(p < 0.05). 하악 제2대구치에서는 치료 기간 동안 후방경사 정도가 클수록 유지기간 동안의 전방 경사 경향이 컸다(p < 0.01). 5. 치료 전의 ANB, 하악평면각, facial height ratio와 유지기간에서의 하악 중절치 및 대구치의 안정성에는 상관관계가 관찰되지 않았다. 6. 치료기간 동안(T1-T0) 하악 전치부 치조골은 하악 중절치 CEJ 하방 6 mm와 9 mm 지점의 설측 치조골이 얇아졌고(p < 0.05), 설측 치조정의 높이도 낮아졌다(p < 0.05). 유지기간 동안(T2-T1) 하악 전치부 순측 치조정의 높이는 낮아졌다(p < 0.05). 7. 치료 종료 시 CEJ 하방 6 mm와 9 mm순측 골 두께가 얇고 순측 치조정 높이가 낮을수록, 치료 기간 동안 순설측 치조정 높이 감소량이 클수록 유지 기간 동안 하악 중절치의 전방 경사 경향이 컸다(p < 0.05). 치료 종료 시 순측 치조정의 높이가 낮을수록 유지 기간 동안 하악 평면에 대해 하악 중절치의 압하 경향이 컸다(p < 0.05). 이상을 종합하여 성인에서 미니스크류를 이용한 골격성 Ⅲ급의 절충치료 시 하악 전치열의 원심이동이 효과적으로 일어나며 치료 결과 또한 비교적 안정적으로 유지됨을 알 수 있었다. 특히 교정 치료 완료 시점에서 하악 전치부 순측 치조골이 얇고 순측 치조정의 높이가 낮은 경우 그렇지 않은 경우에 비해 하악 중절치의 장기적인 보정이 필요할 것이다.

Skeletal Class Ⅲ malocclusion is a relatively common form of malocclusion in Korea. Especially in adults with no potential growth, either orthognathic surgery along with orthodontic treatment or camouflage orthodontic treatment is carried out. In borderline cases where only mild skeletal discrepancy exists and if worsening of the facial profile is expected as a result of premolar extraction, mandibular full arch distalization with miniscrews is the treatment of choice. The purpose of this study was to investigate the pattern of tooth movement and evaluate the stability of mandibular full arch distalization and to identify correlation between stability and factors such as initial skeletal pattern, dental changes during treatment and alveolar bone in symphysis region using lateral cephalograms. 17 patients who were diagnosed with skeletal Class Ⅲ and treated with distalization of mandibular dentition with miniscrews on buccal side were selected. Pre-(T0) and post-treatment(T1) and retention(T2) records and lateral cephalograms were retrospectively collected. Horizontal and vertical positions and angulations of the lower central incisor and molars and the width and level of alveolar bone in symphysis region were measured and the results were as follows. 1. Horizontally, during treatment (T1-T0) lower central incisor (p < 0.05) and 1st and 2nd molar (p < 0.001) were significantly distalized and during retention (T2-T1) no significant change was observed in the position of the lower central incisor but lower 1st and 2nd molar moved mesially (p < 0.05) with the relapse rate of 26.8 %, 23.9 % respectively. 2. Vertically lower 2nd molar was intruded during treatment (T1-T0) (p < 0.001). No significant change was observed in the lower central incisor, 1st and 2nd molar during retention. 3. As to the angular change, during treatment (T1-T0) lower central incisor (p < 0.05) and 1st, 2nd molar (p < 0.001) tilted distally. During retention (T2-T1) no significant change was observed in the angulation of the lower central incisor and 2nd molar but 1st molar tilted mesially (p < 0.05) with the relapse rate of 24.8 %. 4. Lower 1st molar tended to move more mesially (p < 0.01) and intrusively (p < 0.05) during retention as it moved more distally and extrusively during treatment. Lower 2nd molar tended to tilt more mesially during retention as it tilted more distally during treatment (p < 0.01). 5. No significant correlation was observed between initial skeletal pattern (i.e. ANB, mandibular plane angle and facial ratio) and stability of mandibular molars and central incisor. 6. As to the alveolar bone in symphysis region, lingual bone thickness at 6 mm and 9 mm apical to lingual CEJ were significantly reduced (p < 0.05) and lingual alveolar crest level went down significantly (p < 0.05). During retention (T2-T1) the level of labial alveolar crest decreased (p < 0.05) 7. Lower central incisor tended to tilt more anteriorly during retention as labial alveolar bone at 6 mm, 9 mm apical to CEJ was thinner at the end of the treatment (T1), as labial alveolar level was lower at the end of the treatment (T1), as reduction in the labial and lingual alveolar crest level was more during treatment (T1-T0) (p < 0.05). The lower central incisor tended to intrude more during retention as the labial alveolar crest level was lower at the end of the treatment (T1) (p < 0.05). The results indicate that the distalization of mandibular full arch is effective in distalizing the entire dentition and the results are stable. Especially if the labial alveolar bone in symphysis region is thin and the level of the labial alveolar crest is low at the end of the treatment, longer retention is required for the outcomes of the treatment in the lower central incisors to be stable.
