Analysis of the kinetics in the foot play an important role in understanding foot and ankle injury and degenerative diseases. In order to measure the in vivo joint forces, an artificial joint with a built-in force sensor must be installed by an invasive method. There is a disadvantage that an orthopedic procedure is necessary to measure the joint load using a force sensor. Unmeasurable in vivo joint kinetics can be estimated by a musculoskeletal simulation method. In this study, we developed a foot model that can analyze the foot kinetics of normal feet and feet with pathologic disorders using a dynamics simulation method. We also predicted abnormal biomechanical changes after surgery. In the first study, a multi-segment musculoskeletal foot model was developed for clinical gait analysis. Active and passive moments were calculated separately along with the net moments in the foot joints of normal people. The predicted results were evaluated through comparison with previous research results. Separated active and passive moments can be used as an accurate clinical diagnostic tool for disease. In the second study, the kinetics of flatfoot subjects during walking were analyzed. The gait analysis data of the subjects with flat feet were obtained and the differences of the active and passive moments between normal and flat feet were quantified. The net moments of the flatfoot subjects decreased significantly in the foot joints. Active moments were significantly reduced in ankle joints, while passive moments were significantly reduced in Chopart's joints. The results of this study are expected to be useful for the development of treatment methods and understanding the causes of disability of flatfoot subjects. In the third study, changed kinematics and kinetics in foot joints during walking was estimated after subtalar joint fusion that is recommended surgery for alleviating pain in the flatfoot. The multi-segment foot model developed in the first study was used to calculate joint contact forces in normal and subtalar fused foot joint and the calculated joint load of the normal subjects was evaluated by comparison with the results of previous studies. To mimic subtalar joint fusion, relative motion between the calcaneus and talus within the musculoskeletal model was constrained. The aggravation of arthritis after subtalar joint fusion was identified. The load of foot joints increased significantly after the subtalar joint fusion. In conclusion, a five-segment musculoskeletal foot model was developed, and human internal kinetics were estimated in the flatfoot and subtalar fused foot. Moments including net, active and passive and joint contact forces in the foot were estimated. Quantified kinetics of normal and pathologic walking can be used to help decision making for the treatment of foot deformity and suggesting optimal treatment strategies.
